I've never had this problem, but since updating to 1.4.6, my FPS had dropped significantly from being 70-85 to 35-45 with occasional lag spikes. My settings before and after the update did not change (using optifine) and were as follows -
Far render distance, max FPS, fancy graphics, chunk loading default
Specs -
AMD Dual core A4-3420 accelerated processor
4GB DDR3 RAM
AMD Radeon HD 6410D integrated graphics
Windows 7 64 bit
Tried -
- Optifine doesn't help, seems to make it worse
- Allocating more RAM doesn't help
- Switching to normal render distance doesn't help
- There are two Java files installed - Java 7 update 6 (64 bit) and Java 7 update 10
Try only having java 7 update 10, I also updated to it and even before that I had the same problem with low fps and have 2 gb ram.

Check to make sure your AUDIO drivers are up to date.
MC has been having weird audio issues in the last few updates, and they do cause game lag. It's worth a shot, and audio drivers are often overlooked by gamers for updating.
Thanks BGG, I'll take a look into that. Also, the Mojang team is still working on the rendering engine rewrite, which can lead to some pretty horrible bugs.
