1.4.6 fixes the visible stutter, but not the audio problem or lag. This is a result of people filing a bug which covered multiple topics (minecart stutter and flickering entities), Mojang not splitting it into separate issues, and then compounding the problem by deleting any clarifying bug reports as "redundant." So for now we're still stuck with the more troublesome part of the problem: the laggy / audio stutter.
Same problem here. The visual stuttering is gone with 1.4.6 but the sound is still bugged. Friends of mine have the same issue. The fix with exchanging the openAL32.dll didn't help.
I exchanged the files inside.ogg and base.ogg for two ogg-files that contain just silence and put them on write-protection so minecraft can't restore them. Now I have silent minecart rides, which is at least better than the stuttering.
Great suggestion. Do you see a difference in lag without the sound?
I seem to notice less lag now under 1.4.6, but I'm not sure if it's placebo effect or not. But the sound is annoying.
Lots of other reports of this problem, but Mojang is deleting them as duplicates of a bug which isn't really addressing the problem. As a result, I don't think we can expect much movement on this. Dummies.
Here's the "main" bug report if you want to add your weight to it:
I have the same problem running 1.4.6 (had it on earlier versions too). I never did have the problem in Alpha/Beta/etc... I think it only cropped up around the time of the new minecart sound.
I am using a Linux, i7, 8GB RAM, Nvidia Graphics machine. Everything else runs flawlessly. I don't mod other than MCPatcher for texture.
It did crop up with the new minecart sound, you are right. On the first release, there was a bug where the sound would not stop even after you exited the cart. They fixed that.
I think the problem is twofold: The minecart sound runs for an extended period of time, in a loop. There aren't too many other sounds that run in a loop. the last time we had a problem like this was with rain, which also rain a looped sound and caused horrific lag, to the point where people turn weather off entirely.
Secondly, the guys a Mojang are trying to play with a sort of doppler effect with the sound, having it increase in volume and change pitch a bit as you go faster. If you notice, the sound get stronger when you go downhill and pick up speed, and the pitch changes. This may be causing problems as well. The fact that they are OGG files isn't helping either.
Hope so. Mine started bugging out a couple of days ago. Whats werid is it happens leaving one base ,then everthings fine until i get close to another base.
I'm having the same issue, but I think I noticed something else. It seems to happen when another minecart is in the same y coordinate as the minecart your in. When I remove all but one single minecart, it doesn't happen. What does happen is that my automatic rail system glitches out when the minecart goes a long distance. When the minecart is going around the track and it's returning to where I am (far from the point of origin), it glitches on an off the rails and goes through the walls and stuff. it doesnt even let me get on the cart cause it's not even on the rail, but then suddenly it just keeps going. Sometimes it doesn't happen when I'm riding a cart but that horrible sound and those glitchy visuals are so annoying. I have no addons installed either. Just the plain Minecraft client.

UPDATE: not true. Still getting lag spikes under 1.4.6 which seems related to the buggy sound playback.

This looks like it will be the new "main" thread on this issue: https://mojang.atlassian.net/browse/MC-1642

You'll be happy to note that it is reported as Resolved for 1.5 today: https://mojang.atlassian.net/browse/MC-1642
Now, we just need to fix glitching mobs:
