I don't think you're losing items, I think they are combining together on the floor. I've noticed this behavior since 1.3.2. Notice how there are 4 items loosely associated with each of the 4 floor blocks the items landed on?

To test, empty your inventory then count blocks mined and then count after they go into your inventory.
This is really starting to become annoying. When I'm mining I notice that sometimes the dropped item entity just flat out disappears.
Example: When I finish mining this I should see 6 stone blocks and two dirt blocks.
But as you can see, I don't. I didn't pick them up, else I would have picked up the rest as well.
I notice this happens more often (I think) if I hit the entity in the few milisecs after I break the block, but some will still disappear whether or not I hit them. Please tell me this is going to be fixed soon or even better there already is a fix.

Hmm. You were right. Strange, I've never seen dropped blocks sync so well. Thanks for the tip :).
they changed the code in 1.4 i think so it would pretty much be a single item rather a bunch to reduce lag etc (i know thats not exactly how it is done, but makes the most sense)

I think this was a minor measure meant to reduce game load. Instead of rendering several items that are all overlapping in the same general area, the game combines them and only has to render one item.
Example: When I finish mining this I should see 6 stone blocks and two dirt blocks.
But as you can see, I don't. I didn't pick them up, else I would have picked up the rest as well.
