When you jump onto a ladder and keep sliding sideways until you fall off, regardless of you holding "A" or "D" to counteract the direction you are sliding and "Shift" to prevent yourself from falling off a ladder.

Are you currently using a snapshot or 1.3.2?
If this is a snapshot, than we can't really help, as its a bug. If its in 1.3.2, it could be your movement keys aren't WASD, I suggest checking your controls.
