7). Do you have a decent microphone/headset: Yes. I'm using a Blue Yeti, but it tends to not sound as good on Skype calls then it does in recordings.
8). Can you record: Yes, I have a channel and that's one of the reasons I wanted to apply.
9). Can you host the server: Technically yes, but I can't guarantee a great connection. It varies from state-to-state.
10). Are you up to doing other games then minecraft: Depends on the game really.
11). If you play other games, what games and genres are your favorite?: I'm really a FPS/Action player, i.e. Skyrim, Halo. But I like sandbox games like Garry's Mod.
