Cool But I personally believe in "minecraft lore" that a cult of weird-ass humans opened the portal to the Ender realm.. which then Endermen came through, and ravaged all kind of ****.
The endermen came from a realm beyond the end; they came through an escape portal they built from the dragon realm. One dragon slipped through with them and they fight a constant battle against the dragon. Then every now and then a human opens a portal to the end realm and some enders slip back through and end up stranded in the overworld.

I wrote up a document regarding how the Nether came to be. It comes from more of a "minecraft religion" approach and is more supernaturally, but don't let that deter you from it. The link is in my signature.
This was written during the beta 1.9 update, so it may not contain all the latest stuff. Also, I've been rather ambiguous with it, but I've explained a few things in the comments.
What about the end itself?
My theory: In the beginning there were two parallel universes. In one the endermen and other odd creatures roamed, in the other pigmen, creepers, slimes etc.
Anyway the endermen were similar to pigmen, only brighter and lacked morals. The Endermen created a liquid weapon called the shroud (a portal that mixed the void and water) and used it to destroy cities. They lost control over it though and it slowly spread over the land and up through the air. It was like acid that consumed everything,. Realizing their entire universe was doomed, they created stone islands and protected them with ender crystals. They stayed, covered in shroud, for a 1000 However they realised that the end result of the shroud was a void. They lived forever on the islands, for time didn't work properly. Eventually they started looking for a new home and found it. They called the islands void-ships and named them, each unaware of the others existence. Eventually they started moving them around and despite the fact the void was endless they found each other. Then they started looking for a new home.They were corrupted by the void and . One by one the everlasting endermen went insane, destroying the void-ships until only one is left, ruled by one of the other native creatures of the other world who is just as insane, corrupted by the void and even more powerful. The void-ship is called the End. And it is there that the pigman's portal opened.
