Average is like 30 or 35, I think, so 45 is definitely not much more. I actually thought back when I typed about 80 WPM that was I was pretty average typist, maybe 60, so I think it's a lot to do with perspective. I know it certainly surprised me when I found I could type almost 100 WPM.
Typing one's own thoughts, if they are flowing, is probably faster since you can look directly at what you're typing and you don't have to process reading the text at the same time.
I personally type the proper way, with all 10 fingers. I'm a programmer by trade so fast efficient typing is important. When typing out text from my own thoughts I average above 100wpm. When copying text like on that speed test site, I do between 85-95 wpm on average, depending on the complexity of the text (ie punctuation and capitalization as well as unusual words or spellings).

I get around 75 words per minute... you should add that to the poll (as another question with like 0-29, 30-59, 60-89, 90-119 words per minute.

Usually, I use all 10 fingers. I don't use my little fingers much though, only when I need shift or a ' or something like that. In elementary school, they taught all of us how to type correctly. Sorry if this sentence seems really dry and emotionless, I'm trying to hard to count which fingers I use to type.
Every computer class I took in Highschool started off with learning how to type properly, so I got it down pretty good. I can usually hit around 75 WPM.
It's also kind of funny to look at people and talk with them while continuing to type (i.e not looking at the keyboard or the monitor)

I like to do that, and since I am the only one in my class that can type more than like 40 WPM, they always wonder how I don't need to look at the keyboard and still get in the 90's for my WPM
Edit: Also, I think I am one of the few people in my class who uses "Capslock" for all capitol letters, never Shift.
I can type with around eight, but I have kind of a different layout...

Yeah I type with most of my fingers but rarely use the pinky and the one next to the pinky (ring finger?) on my right hand. So in general I use 8 out of my 10 fingers to type.
Scratch that, I press the enter button with my ring finger, which is pretty important, I guess that makes it 9, haha.
But I think I type pretty proper, 79 words per minute isn't half bad...
I can type pretty fast even though they dont teach it at my school I think it is one of those things you need to learn by yourself everyone is different even in the smallest regard about how they type.

I can type with my eyes closed too, in fact I'm doing it right now.
