How: Someone threw a gernade of a mancannon, it hit a wall bounced off a little and stuck me on the head. I then contiued walking before i died and hit another player, thus making it a double kill for one stick.

how: got chased by a helicopter shooting bulletd at me while i was in a red corvette and then the engine ignited forcing me to get out or die so i got out and then the helicopter chasing me decided to swoop down and spill my guts with the copter's blades

Got to (I think) chapter 8 with the part in the transit hub and you have no air while Eli tries to open the door (can she take any longer to do it?) and on of those necromorphs with the leg guards comes charging at me. I continuously forget it's not a good Idea to try to shoot their legs off since it takes too long, so I try to shoot it's legs off witht he plasma cutter. I have two damage upgrades and the special fire upgrade and it's legs don't come off after four shots. Hits me once and for some reason, I get stunned. So, it hits me again and I get stunned, again. Then, it hits me a third time and I die when I had full health when he hit me the first time and two health upgrades. I also hadn't used my first save yet since I was planning to do it on chapter 10. Oh, how I hate hardcore, but it's the last achievement I need and I want the hand cannon.
How: Finished the final touches on my "Magma Cannon." The problem was that I didn't test the pumps prior to connecting everything. Magma went up 60 Z-Levels, sprung a leak and covered the shutoff switches/cranks. Magma then filled my fortress and slowly began flowing downstairs, roasting the lower levels one at a time.
So I'm a level 10 Wizard. I have 46 hit points, which isn't very much. My armor class is 19, which also isn't very much. This is typical of a Wizard. Wizards are supposed to deal damage, not take it.
So our party enters a room with 8 skeletons. They don't immediately attack us, so we take the time to cast some defensive spells on ourselves. These skeletons, controlled by the spirits of experienced warriors, take this time to talk strategy and draw their bows.
I win on initiative. In fact, I was the only one in my party to roll well on initiative, and I beat all the skeletons, so it's my turn first. I attempt to end the fight right then and there by letting loose with an empowered Fireball. I deal 64 damage (quite a lot of damage) to each of them, but none of them fall. I end my turn, a bit disappointed.
All of the skeletons did better than the rest of the party on initiative, so they go next. Their target is obvious: Me. Each of them gets two attacks each round, and so I become perforated with about 10 arrows, 1 of them a critical hit. I go down. Our DM keeps track of life totals for us (he announces damage to us flavorfully), so we're not sure just how dead I am. The Cleric attempts to use Breath of Life on my dead body, healing 39 life, in an attempt to resurrect me. Had this brought me above 0, it would've worked, but it didn't. The DM then announces that I'm still at -69HP.
I later learn that each of the skeletons had less than 10HP left, so I actually very nearly one-shot the lot of them. That made it all the more painful.
So anyways, at level 10, death isn't a big deal. It's more of a monetary inconvenience than anything. I get resurrected, and we continue with the dungeon. Shortly after, we reach the big-bad of the dungeon: Armag, A barbarian-rogue gone mad with the power of the gods. He is flanked by 8 skeleton warriors.
I do well on initiative once again and attempt to lock the big-bad in an icy-prison. He not only makes his save, but his sword, an intelligent artifact, successfully casts a counter-spell. Having wasted my time figuring out I can't touch him, I end my turn.
At this provocation, the big-bad charges us. Thankfully, our melee people are in front of me and thus take the brunt of his blows as he performs a great-cleave on the lot of them. Our melee people retaliate by getting him into flanking, but it's useless because you can't flank someone with more rogue levels than you. Two of the skeletons charged into melee, the rest whipped out their bows and targeted not me and did so very unsuccessfully.
So, feeling like I can't do very much to the big-bad, I figure I can start picking off the skeletons using Ball Lightning. I'll get out two castings of it and move the balls from skeleton to skeleton, murdering them, while the rest of my party deals with Armag. I get off one casting, and it works well, killing a skeleton, until one of the skeletons hits me with, surprise, an arrow. It's a crit for 42 damage and also my lung is filling up with blood. I'll start drowning after a few failed fort saves.
As our meleers continue to battle with the big-bad, I figure that I don't have much time left before I'm unable to cast any more, so, not learning from the mistakes of my past, I throw a fireball at the group of skeletons. None of them die.
By the time it becomes my turn again, I'm at -118HP.
In game-time, I was resurrected only 3 minutes ago.

Damn. I remember a looooong time ago when I was trying DnD for the first time, my friends convinced me that beholders were weak and that my Sorceror could solo him. To say that I was totally obliterated would be an understatement.
How: Chrome Pyramid (think unstoppable god monster) teleports on top of me and pulls an insta-kill with its Shotgun Missile Launcher.
How: Not looking when I jumped off a ledge.

Uninstall the game for the sake of your computer.
