Their appearance isn't scary at all, the way they attack you is though. They constantly jump out of vents groaning and yelling and are harder to kill than cockroaches. I swear, I never leave aim mode when walking in a dark area because I know that one of these shits will be in there waiting to ambush me.

A. Dead Space Lurkers. Deformed mutant babies.
B. Goddamn Dr. Salvador from Re4.
Reasons?
1. Almost impossible to kill with a handgun.
2. Really Fast.
3. One hit kill.
4. That noise he makes.

First time i saw him i was like wtf? he gave me the creeps. he still creeps me out. zelda ftw
The Doctor( or whatever his name is) from Penumbra. Yeah, I know you can out run him and lock him in that room, but he's just plain creepy when he first pops up.
Redeads from Ocarina of Time, hands down. What other enemy screams at you, freezes you in place, and then leaps on your back? Close runner up from the series are the Like Likes...things seriously creeped me out whenever I faced them. Want to get close? TOO BAD.

That awesome soldier you've been training since the start of the game? Yeah, he just got brain sucked. There's no way to save him and he's now aiming a rocket launcher at the rest of your team.
I've never understood the fear of ReDeads in OoT...then again the things I fear in video games are much less understandable.
The only thing I've feared in a Zelda game was in Link's Awakening, the owl. The fact that it always spoke in caps scared me for some reason.
The only common one I can think of would be the guards in Oblivion, whenever I stole something in that game I almost always expected some guard to pop out of nowhere and scream his head off at me. Sometimes it did happen, and I promptly screamed at the screen.
The uncommon one would probably be "Black Widow Queens" added by Martigen's Monster Mod in Oblivion. They give off blue smoke like dread zombies, they have little black widows all around them, and more often than not they spawn with another black widow queen and they're about twice as large as the player character. Just the sight of them sends shivers down my spine. Oddly enough I don't find spiders in Minecraft that scary.
Poison headcrabs and their corresponding zombies in Half-Life 2. I never finished the Ravenholm level because I kept getting so freaked out. (Well, that and because the thing autosaved when I was out of ammo in a location where you've got maybe three or four seconds before a bunch of baddies show up.)
