1) You have to pick rank you want to disguise
(then you have to click on "set your rank")
2) You have to pick your name
3) You have to pick your tab prefix.

It's a nice concept - however, very few servers use GroupManger due to how limited it is, and even fewer use PEX now that is it no longer supported in 1.13.1. I highly recommend you looking into LuckPerms! It's been the biggest replacement for all group management plugins.
