I have a fear of cave spiders. I haven't been scared by any other mob (excluding bosses), but I'm scared of cave spiders for some reason, however, regular spiders don't give me the same feeling as cave variants do. Every time I find Mineshaft fear strikes because of them, so when I finally find their spawner, I escape back to the surface. Once I managed to get there and make XP farm, but that was a lonely exception. When I see the cave spider I start to panic. I need help
Not me; I even made them naturally spawn everywhere underground, as suggested by their name (as opposed to "abandoned mineshaft monster spawner spiders", as somebody else called them; you'd have thought that a "cave update" would have made them true to their name, or at least spawn in more areas than ever-rarer mineshafts (they were made 2.5 times less common in 1.7 and 1.18 effectively made them half as common due to twice as much ground, thus compared to 1.6.4 there are only 1/5 as many mineshafts for a given volume).
They are also very easy to kill, at least in 1.6.4 (a Sharpness V diamond sword deals 14.25 damage, killing them in one hit (12 HP); since 1.9 you need Bane of Arthropods, of which you don't need much to one-shot them, and it also inflicts Slowness IV), with most of the damage they deal coming from the slow Poison effect, which can be reduced with Protection enchantments and otherwise gives you time to get away from danger (you can also drink milk but that is not practical if you spend hours at a time caving like I do; once you reach late-game you could afford a shulker box full of buckets, with one or two ready to drink in your inventory). I also use shears with Efficiency V to clear away cobwebs faster.
It is also ironic that despite all the new (hostile) mobs added since 1.6.4 you usually won't encounter them while caving as they either only spawn on the surface or in special structures; other vanilla mobs that are lacking are silverfish, which I made naturally spawn (only those from infested blocks or attacked by a player can hide in blocks), endermites, which I made spawn naturally and when an endermen teleports (a feature in a snapshot which was removed by the full release), and husks and strays, which should also spawn underground, not just on the surface (I made them 50% instead of 80% of zombies/skeletons to account for this; I also added more husk and silverfish variants to match the biomes they spawn in; desert = yellow, mesa = red). The addition of naturally spawning witches in 1.7 was also a nice change, as I'd literally never seen a witch until I added them myself (they always despawned immediately during world generation as they were not persistent until much later, and aside from that I don't make farms and the chance of a witch spawning in such a small structure in the short time I spend passing near one, and only at night, is insignificant).
Are you afraid of spiders in real life? I know you said you aren't afraid of the common spider variant but I still wonder if it's related.
Are you also afraid of the wither skeletons or witches? What about blazes or lava? trying to possibly find a cause and wondering if the poison or damage over time is part of it.
What about baby zombies? They are small and harder to hit.
Maybe the combination of being rare, smaller, harder to hit, and dealing damage over time does it.

I'm scared of stuff I don't know how works and/or have seldom tried.. Once you get experience fighting something, and you get to the point where it usually works out fine, I might up being way too casual fighting stuff Though hardcore mode helps to keep me on edge..

The nether is actually safe(ish) to a degree. Notably once you have a gold piece of equipment.
In the nether, if you're wearing something gold, you keep piglins neutral (unless you dig gold near one). Piglin brutes are an issue, and they are limited to bastion remnants.
Hoglins can be an issue, though are limited to crimson forests and bastion remnants.
Zombified piglins are neutral until you hit one.
Enderman are sparse to one biome (or very rarely outside it, it seems) and are neutral until look at one.
Magma slimes are sparse and slow, and in their smallest form, don't hurt you.
Ghasts are a potential issue but a bow or running (or just avoiding the projectile) makes them easy to manage.
Wither skeletons are limited to fortresses, and rather easy to manage with a bow or shield. Same with blazes, and fire resitance potions almost negate them entirely.
The wither itself needs specifically summoned (and isn't a nether only or spawn case anyway).
Of course, like many things in Minecraft, it's often never just one thing that gets you. It's "the ghast shot near me and then I fell into lava" or I took a hit from a piglin or zombified piglin I hit, then ran and took a fall and died shortly after". So I know reducing everything to sounding like nothing doesn't mean there's no risks. The nether definitely carries risk. The collective whole has a lot of possibilities. But outside ghasts, wear something gold and you're... golden? Haha. Almost everything else is either passive, a low threat, or limited to where you find it.
The overworld, by comparison, is only safe during day, and only on the surface, and even then only in spots, as multiple mobs that attack you on sight are all over and can spawn in complete darkness (mushroom islands and the deep dark biomes aside).
I even fear the end (until I get elytra, anyway) more than the nether.

Why do I still play in 1.6.4?
Cave spiders are small and fairly deadly in the early game with their poison and climb. It's pretty natural to be afraid of something like that.
