I agree, I'd rather use the diamonds for a diamond sword. Iron hoe seems like a waste as well. A wooden hoe is all that's needed, or a stone one if you have a huge field of wheat.
One iron hoe will last you a lifetime and for the low low price of only 2 iron ingots!!!

You just place them, they do nothing other than show "i beat the boss" which pretty much everyone has done, at least once in their MC gaming time. They annoyingly move around if you clik them.. that's it..
Diamond hoes are useless, i agree. Who wants to waste 2 diamonds for something that can be easily done with wood/stone? You can just make more wooden ones if they keep breaking, nothing changes if you use a diamond one.

A fire charge is up there with them. Unless you are caught in the Nether, there is little reason why one would use it. The destructive capabilities of fire have been nerf'd almost beyond the point of being usable, and a fire charge can only start one fire. Even as a projectile, arrows can be cheap and easy to shoot through a fire.
I think the most useless item is...the silver fish spawner egg, since silverfish don't even give you anything and they're an absolute pain in the assHoe dear! Why does everyone think the hoe is hoerrible? There is still hoepe that the hoe can be hoelpful. I have one in my hoeme.
It's recently occurred to me that leather armor is almost completely worthless.
It barely gives any protection, for one thing. Iron armor gives far better protection and, honestly, 24 iron ingots is not that much for a nice set of armor.
There's not that much else you could use the leather for, but using it for bookshelves to power up enchanting tables would be a better usage of it. Mind you, there are mods which make leather more useful; those make leather armor even more of a waste of materials! If I could upgrade one of my backpacks, why the heck would I make a suit of worthless leather armor?
Even though it's not the most useless item (well, set of items in this case) in MineCraft, leather armor is pretty much useless.

Nothing is actually useless. Even leather armor provides decent protection very early game. But the least useful overall is probably clownfish. It's just a rare catch and nothing else. Raw fish is more common, restores more of the hunger bar, and can be cooked to restore even more hunger. Raw fish can be used to tame ocelots too. There is literally no reason for clownfish to exist other than as a trophy.
Bit of a redundant topic, but I'd have to say there are certainly a whole host of items that get dubbed "useless." Different items have different uses for different people. I'd personally have to say dead bushes seem useless to me, unless you decorate with them. There is literally no other use for them. Yet.
-Sponges: Slightly useful if you need to empty a large area of water, but I personally have yet to experience a scenario where I need to drain such an area. Otherwise, it's useless.
-Dead bushes: Atmospheric value in deserts, and nothing else.
-Dragon Egg: It does nothing but look pretty.
-Beacon: Requires a large amount of iron to actually work, has a short range, and only one of the effects is actually useful by the time you can get one (that effect is haste, by the way).
-Diamonds: No, really. Iron is OP in Minecraft, and diamond is just overkill and has few uses outside of tools unlike every other tool material in the game. All it can do outside of pointlessly powerful tools is make enchantment tables to make your pointlessly powerful tools even more broken, make pretty fireworks that don't do anything, and make record players. I don't see why people value it so much, its best use is picks that don't break in fifteen minutes.
-Brewing Stand: I'm really, seriously not trolling you guys by listing three of the rarest items in the game. But most of the effects are worthless, the exceptions being speed (which is just a minor convenience), night vision (again, a convenience, although it's practically a necessity for raiding ocean monuments, even moreso than water breathing) and fire resistance (which I personally almost never use since buckets of water are pretty good at preventing firey death from lava, and even if you can't use water buckets a bit of carefulness will prevent your doom).
-Clownfish: Pathetic hunger restoration and nothing else.
-Pufferfish: Abysmal for restoring hunger and makes water breathing potions, one of the least useful potions in the game (only good for large construction projects underwater and ocean monuments, the latter of which is easily handled with Respiration and/or signs and doors).
In order, i'd have to say Pufferfish<-Dragon Egg<-Clownfish<-Dead Bush<-Beacon<-Sponges<-Brewing Stand<-Diamonds.
