Even with this computer which has Intel HD graphics I can run fancy without problems. Plus all the other graphics settings at highest, still not much lag.
I play on a laptop. More specifically, a fairly low-end laptop. The graphics card is terrible (it's the kind that most non-gaming laptops have), so it can't handle fancy graphics.
Even with OptiFine, it can't handle fancy graphics. Mind you, OptiFine does allow for selecting individual fast/fancy settings, so I usually play with water, clouds, and grass on fancy and everything else on fast. (I also use OptiFine's "clear water" setting, which makes it much easier to see underwater.)

Its not graphically demanding on any graphics card. its not the GPU itsself that causes low fps, its the drivers and software for it.
You can get great frame rates with a crappy card and a good processor, and you can get terrible framerates with a great gpu and crappy CPU. I've noticed having current drivers and updated Java do wonders for pretty much every system, and the game seems to depend exponentially more on the processor than the videocard. Mipmap and Anostropic filtering are where your graphics card comes useful.
Currently I'm using a $350 laptop with no graphics card. I'm playing at 60+fps at 1080 on my secondary monitor with fancy and max render distance using only integrated graphics on a mobile i5 with only 8gb of system ram. sure I don't have mipmap or anostropic filtering, but the screens I play on have high pixel density and I don't find those settings improve the look at all.
I generally use fancy on singleplayer but if I get even a tiny bit of lag I switch to fast, and I always use fast on SMP
