End portal and End portal frame - 27 end portal blocks per world and 36 end portal frame blocks. Possibly less because of terrain generation.
I'd have to say emeralds, though I've found a decent amount of them in my extreme hills biome so far. (I have around 30 total; a lot of it is from a Fortune III pick though...)
Because they can only spawn as one emerald ore per chunk, and extreme hill biomes are fairly rare, they're definitely a very limited resource unless you can find a village with good trades.
EDIT: Oh, and the stone bricks with the circle patterns, as well.

This depends way too much on your play style (Miners will have more diamond, Adventurers more circle bricks, mechanics will have used up more red stone etc...)
It also depends on the version you started in (Ice, no ice, ice again, cocoa beans, etc..)
And then there is farms.. Without my blaze farm I am pretty much sure blaze rods would be rare in my world.
All I know is the blocks I am holding on to. Like the old wooden slabs. Every day they are becoming rarer by time, gnagna...
For me Mycellium and Ghast Tears. Mycellium is tough to find, and it's tough obtaining a ghast tear after killing a ghast since it oftenly falls in the lava.
