Rarely ever do I die by the hand/weapon/explosion of a mob, and when I do it's early in the game. Falls are less common than that, and lava/fire deaths are extremely rare because I always have a water bucket on me. I had no idea that about 1 in 3 voters on this poll apparently didn't keep a water bucket in their hotbar whenever possible, it's one of the most useful things in the game.
However, when I do die, which is rarely, lag does me in. 95% of the times I die, it's because of lag. It infuriates me.

I use the mod Better Archery, it adds new rapid-fire bows for the skeletons to use, they are insanely fast especially within a few blocks of you. When you're at low health or without armor, those bows can be deadly if I am unlucky enough to come across one.

a water bucket doesnt do anything unless you fall in 1 block deep lava lol. you need a fire resist potion in your hotbar to avoid lava deaths.

yeah, and then you are stuck in the lower layers because you fell into a pool deeper than 1 block. also, water does nothing in the nether. good luck placing the water on the top layer if you are in a lava pool deeper than 1 block. water is good for caving, terrible for escaping lava. enderpearls or fire resist potions are the way to go.
Falling has killed me most of the time so far, holes in the ground I couldn't see until it was too late being the main culprit there. I don't die a lot though, these events are few and far between.
Lava has also gotten me a few times. But now when I'm around large lava oceans deep underground I make sure to have a fire resistance potion in my "panic slot", the few times I have had a water bucket there it hasn't done diddly squat to save me when I did fall in, plus water doesn't work in the nether.
But mobs? I don't remember the last time I died by their hand, even after I started playing on Hard difficulty. Creepers can some times sneak behind me when it is too noisy to hear their otherwise obvious footsteps from a distance but the iron armour minimizes the damage. Skeletons are easy to deal with by luring them to the nearest corner, building a 2-tall block cover wall or just shooting them from a distance with the bow. Zombies can only be a moderate threat in massive numbers but running to the nearest choke point is usually enough to be able to kill them one by one as they come within melee range. Witches however, are a worthy opponent, they are dead silent so they can not be distinguished from creepers (besides the speed of the footsteps maybe) and their potions go right through my armour and can bring me to half a heart of health, slow me down, etc. I have had several close calls when they manage to surprise me or decide to drop by when I'm dealing with other mobs.
