I am pretty sure that ender chests are player specific, so just give each new player an ender chest or have them make there own. Then noone could steal items. I am assuming that you only want the vault as a means of item protection.

The OP of this topic went silent and quit updating his post so that design was out of date last I knew. There are some other good designs that some have posted though...I'll try to find them for you.
Forgive me if this has already been submitted as a design or idea, but I have a fairly good 'un. Instead of a door, you have a minecart and a 1x1 gap. If the code is entered incorrectly, the cart is held out of view and you cannot enter it. When the correct code is entered, the minecart runs along the track to the 1x1 hole, the user hops in and away to the vault. As for exiting, a permanent minecart would be put next to the exit.
Sorry if this sounds a bit confusing. I do think this is impenetrable without the correct code, though.
EDIT - Just found a flaw, where the thief or whomever could place a minecart though the 1x1 hole. This could be prevented by some form of piston mechanism which moves the rail that the cart will rest on when the correct code is entered.
I have also decided to make a prototype of this soon I will post it shortly.
The vault is also supposed to be griefer-proof, so if you're using a rail, make sure that a water bucket can't destroy your setup.
So has anyone looked into the nether-portal method recently? I remember it being brought up a long time ago, but with the new dispensers and fire charges... it seems plausible.
Noted that during building :/ I suppose the only way to stop this would be to removed the track from view during closed "mode", but that would mean using breakable blocks. Close, but no cigar Moby!
Don't give up! I'll share some of my own vault ideas (some discussed way earlier), and a large segment has rails. I've found a way to bypass both the problem of water, and the problem of people getting through! This is my vault: (warning, I don't know the specifics of some of the redstone stuff, I'm just presenting the idea)
This is a kinda (ok, really) big wall of text, so for the people who don't like reading's sake, I've put it in a spoiler. It has some good ideas (I think), though!
You have a large room, all bedrock, with a entrance in one side, and an "exit" in the other. The room is a large rectangle, and on its walls is about 20 buttons (random large #). The exit is a mine cart track, with one powered rail to launch you into the room. The ceiling over the mine cart is only 1 high, so you constantly take suffocation damage while in it. The mine cart rail immediately slopes up, so water can't flow in, and you still have the space to place down the few exposed tracks if someone tried placing water. Ignore the buttons for now, just assume someone has either entered the wrong combination, or not entered one at all. They place there mine cart on the powered rail, hop in it, and zoom into the passage. Shortly up the slope, there is a detector rail, followed by a normal rail, then a powered one (so when you go over the detector rail, you don't activate the powered one). When you go over the detector rail, it sets off a slight delay connected to a dispenser. In the mean time, the player is now stuck, unable to move because of the unpowered powered rail (no pun intended). They take suffocation damage, and invertible die (bedrock, remember). At the same time, the dispenser shoots a set amount of arrows at the mine cart, destroying it (remember, it was set off by the detector rail). The slope prevents the player to continue forwards in the track, they can only back up (I think). If you did enter the correct password in the buttons, you pass by the dispenser safely, for the arrows are on a short delay, and the powered rail is on. You pop out of the suffocating mine cart ride with (some) health to spare. You step on a pressure plate (completing an and gate) as you walk into a room. The room is 1x1, and all bedrock except for 1 stone block. You have 5 seconds to log off. At the end of that five seconds, the piston pushes out, and a one in a similar room above it retracts in. The piston is powered through a bedrock block by a repeater, and they are both sticky pistons. When you re-log, you will be in the top room. This is some form of teleporting/elevator thing I forgot the name of. If a griefer does somehow get in with a accomplice, they can't to the same trick manually, for the top piston is inverted, so it opens when the other closes, and the other way around. When you exit the top room, you step on a pressure plate, which resets the entire trap. Also, it is impossible for someone to follow you in, unless they are in the same 1x1 room you are when the piston closes, for they can't brake a block, then place one in there space. When you step on the pressure plate, it not only reset the entire thing, but using a redstone randomizer (the only thing in this entire vault I don't know how to do), it randomly changes the code, which is displayed on the back wall of the "treasure room" using redstone torches. The entire bottom of this treasure room is filled with bedrock to level 1. To exit, you can either use a /spawn or /home, or if that isn't available, use a similar 1 way log off teleporter to a safe location above in a corner of the vault.
Whew! My rant is over! How do you like it? It does solve Mobyguy77's mine cart problem. I hope you like it, and give me some constructive criticism! I love improving my stuff!
Something involving a command block setting people to Adventure mode... Gotta figure it out from there.
Or, you could just use it to teleport people into a room completely enclosed by bedrock, and use another command block to teleport out. All wirings and command blocks could also be completely encased in bedrock.

Why has nobody thought of something so simple: Obsidian surrounded by a layer or two of water, then another layer of obsidian, all underwater, and only accessible from the bottom.
Think about it. If the thief can't find the source or drain quite literally everything, they're forced to try and dig through while underwater. Even if they have a diamond pick, being underwater will drag out that time immensely, meaning that a would-be thief would likely drown before they break even a single block.
Another advantage here is that any break-in attempt will not be very subtle at all, as a thief would be forced to drain a considerable portion of the surrounding water to be able to break just the outer wall without suffocating.
This wouldnt work, because someone could place a sign or sugar cane under the vault to give them air to mine from
If you place a dispenser in the corner like shown, then the item passes through the blocks, and thus there is no actual "vault" to enter, and by extension no worrying about iron doors. Then all you need to do is to make a suitably complex locking system, and there is your unbreakable vault. Do I win?
I built mine in The End. The advantages of mine were:
The Ender Dragon and Endermen could kill almost anything that tries to get to my vault.
Intruders will have to tunnel almost into the void to find my vault.
I created a small cavern under ground, with hidden trapdoors and pressure plates surrounding it. These would drop him into the void.
A triple layer of Obsidian was used.
The trapdoors spread six blocks wide.
The way to get through is a group of levers, which are also surrounded by trapdoors. These trapdoors only open if the wrong combination of levers (out of 9) was pulled.
I used bedrock to create the vault, the vault itself hidden by other bricks. There is only one small gap, made of wood, which looks just the same as the bedrock. The levers set it alight.
The right combination of levers temporarily diffuses only five of the trapdoors. This means you need to choose the right combination of trapdoors as well.
Right outside the vault, after the trapdoors, there is another group of levers. These number the alphabet, each lever representing a letter, and the word 'pass' must be written with them. One wrong lever and every trapdoor opens, causing you to fall into the void.
make a bedrock cube. have a command block designed to teleport a single player, the owner, connected to a button. Does this not work?

This is what I've been working on today (see post here: http://www.minecraft...0#entry22409195). I can't see a problem with it yet (with regards to security). Only problem is that it requires an op to build it and set up the command blocks.
I know this is not the exact goal, but still, i'll post it:
With command blocks, make a security system with /clear and Gamemode 2 swapping, with a warning before the entrance, after that, there is a cube of bedrock with a code lock, when entered correctly, it teleports you inside another cube of bedrock with the vault inside.
