Requires some redstone, but use the Block Door with Sticky Pistons, using a combo input to open the door - all of the wiring is surrounded by bedrock, and when closed the vault is pure bedrock on the outside.
Only way in would be the correct combination AFAIK.
Simple, unless Bedrock can't be moved via stickies...

Was the cobblestone plug idea discarded because of the lag that it causes? Just that?
Now that I'm back from vacations I've been working in a very decent prototype based on the cobble plug model discussed here earlier. Testing it in smp, I couldnt mine the cobble faster than it was pushed upwards, even with a golden pickaxe... nor was I able to explode the entire plug using tnt.
And another question: can you use as an exit method a sand block atached to a sticky piston being pushed towards you, then log-off-in to be teleported to the top of the vault (in the sky limit)?
That's...quite secure, actually. I'm not too keen on the workings of glitches, but I don't think you could get around the door if the passage is only 1x1.
As has been stated about every 3 pages in this thread, minetrack entrances don't work because they can be easily griefed with a bucket of water making it very difficult to repair with minimal griefing effort expended.

Of course, I only demonstrated the water at the entrance. Preventing water at the exit would use pretty much the same concept at the entrance, but would force more damage to the player. Getting damage down to a heart or half a heart would ensure death of the unprepared.
This design itself is kinda messy; I just threw something together to see if it works. If requested, I'll redesign it and send out a map download for others to mess around with.
Unfortunately, this doesn't have a redstone passcode. It can easily be implemented to ALLOW the detector rail being placed to activate the piston. Otherwise, placing the rail won't do anything and the player will die.
Skylinerw, you could change your design so a chest minecart comes out when you get the code right, would avoid damage. (if this makes sense) It would make it smaller as well, i think.
Has anybody thought about a portal inside the vault that is normally unlit, but when a code is entered, it will automatically light, allowing entry via the Nether?

Its withing the last few pages. People kept adding old designs after we finished. A lot of the discussion was about really, really edge cases, involving people logging in and out multiple times and other oddness.

My prototype is finished and working well!!
Tested against known griefing techniques (phasing, TNT, water/lava, ladders, redstone repeaters) I could not enter the vault... And even using a golden pickaxe I could not beat the cobblestone flow, always being half-stuck in the bedrock.
Now all I need to do is a video explaining how to turn on the generator (simple step made by an admin before closing the vault) and how do you actually enter the cobblestone tunnel and your room after entering the correct code (I used Grizdale's lock).
