For the last couple of months I've been playing on a vanilla spigot 1.13.2 server with the difficulty set to hard. Just to clarify, we are using spigot to run the server for the performance improvements but all settings are vanilla and there are no mods or plugins of any type. I built myself a handy little goldfarm on a mooshroom island which seemed like a good place since beyond phantoms (and an occasional drowned near the shore) there shouldn't be any hostile mobs spawning there. On several occasions since starting up the farm I have had random zombies spawning on my island/in my farm with no apparent logic to it. They seem to have a couple certain locations where they 'favor' spawning but all of these locations are well lit and inside well inside of the mooshroom biome by at least 50 blocks. I dug into this, looking for the issue and a possible solution, and I feel confident that this isn't part of a zombie siege since it's only ever one zombie and (at the time of testing) my village on the island was too far away to even be loaded while I was at the farm. I am also absolutely positive this isn't a case of drowned becoming a normal zombie since my shores are over 50 blocks from me. The only other possibility I can see would be in a zombie's ability to summon reinforcements however these are pigmen and the wiki has nothing in their listed behavior that they would summon reinforcements when attacked nor do I understand why they would summon a normal zombie rather than a pigman. Despite these facts I resigned myself to the explanation that it was zombie reinforcements and brushed the issue aside until this morning. For further complicate things and make the issue more confusing today I discovered a zombie in a walled-off section of my nether tunnel that I use to leave the farm. I have never and would not ever willingly attack a pigman in the Nether so beyond the fact this guy appeared in a biome where they shouldn't spawn it is also no plausible that it would've spawned in response to a nearby pigman being attacked since they were never attacked on the nether side of the portal. Just for clarity, the portals from my goldfarm lead to a 'control portal' in the nether that is segregated from the rest of my tunnel and about 15 blocks from my gold farm enter/exit portal. The enter/exit portal has a long hall leading to my main base and it was down this hall, about 20 blocks from my portal (35 blocks from the exit), that I heard the zombie and found him in an old, walled-off area.
Has anyone else experienced this or know how this could be happening? Do zombie pigmen spawn non-pigmen reinforcements when hit and there is no documentation for it? If so, how did the nether zombie spawn with my in the overworld and they dying in the nether from fall/cactus damage? I have built dozens of these farms since you could make 21x21 portals and this is the first time I have ever experienced this. I had never built one on a mooshroom island before but I think it's odd I'm having issues with zombies spawning when this has never been something I experienced before despite where I built them.
I'm unable to address all of the questions you pose, but zombie pigmen are able to summon normal zombies as reinforcements at least through 1.12.2.
The torches on the glass in the donut style gold farms are noted as being used specifically to prevent these spawns.
That's interesting to know, I wonder why that information isn't on the wiki and why I haven't ever seen a zombie spawn from a pigman in all my years grinding gold. It's just so strange that I've never experienced it before and now it seems to occur regularly.
Thanks for the info on the donut gold farm but I refuse to watch youtube videos and just copy a design someone else made so I don't know what you're talking about. If this is in fact due to zombie reinforcements then I should be able to eliminate the spawns with carpet and/or better lighting (I see on the wiki they spawn in light level 9 instead of 7) so I'll figure something out.

I believe reinforcements only happen in Hard, non-hardcore, survival mode, when the global difficulty is above a certain number. Don't take my word on it.

Yeah, reinforcements only occur on hard (that's all I play) and I'm used to seeing it happen with zombies but I have never seen a pigman produce a zombie before. The more I think about it it's probably because I don't engage pigmen outside of my gold farm's kill chamber unless it's the single pigman produced randomly by a portal. All of my previous farms I built in the sky and probably spawn-proofed unintentionally just because of the way I decorate and slab surfaces. The zombie in the nether still confuses me though because I am certain no pigman got hit on that side of the portal and even if one had spawned in the overworld and made it through a portal there is no way he could've made it into the walled-off area where I found him.
Pigmen could summon zombies if damaged by a ghast, or so is my recollection from a couple versions ago, from an episode of ghast hunting where I was careful not to strike a pigman (for obvious reasons).
Running around doing my ghast hunting thing and up comes a zombie, far from any portal.
