I created a portal to the nether world, which works fine. When I return from the nether world I'm not returned to my original portal. A portal is being created about 50 blocks away.
I have no other portals in the overworld or nether world.
Most likely because Minecraft was unable to place the portal in the Nether where it should have gone and so it got placed nearby but far enough away that it doesn't connect to the Overworld one.
If you find the coordinates of the Overworld portal, divide the X and Z coordinates by 8, leaving Y the same and build a new portal in the Nether at those (recalculated) coordinates that should connect to your original portal.
If the recalculated coordinates are at a hard to reach place it isn't as important to get the Y coordinate right but if the position of a portal in the Nether is off by 16 blocks in the X/Z plane it won't connect to the Overworld portal you want it to.

I had to build a stairway and go way out over a lava lake to get a coincident location. It really is necessary though if you are in an MPS world where others have made portals.
