[size=12px]I was playing with my friend on my private server, we found a monster spawner and a desert temple. I was mining some [/size][size=12px]diamonds[/size][size=12px]
I found some diamonds right in front of me (branch mining) and also heard lava. I ignored the popping sounds, mined the diamonds, and lava came pouring in... tried switching to gamemode 1 but my fast typing made an error. I raged and ended up deleting the world...
My friend and I were mining, and as I was walking near lava he accidentally hit me with his pickaxe and I fell in. Didn't loose much though, as we had just started.
Same day, me and that same friend were mining again, and he dug into a cave. He didn't see the 1 lava block in front of him and fell straight in.

Vanilla is the best way to play Minecraft, In my opinion. Plugins suck.

Anyway: I'd found a Mesa Biome several Worlds (and Game-Breaking Bugs, in SnapShots), ago; my first one! I proceeded - since this was Creative, to circle the Entire thing (which took me Many hours XD ) taking 100's of ScreenShots. I finally-settled back into Survival, between a Mesa (Bryce) and the nearby Mesa Plateau F(orest) - a great view with an equally-good vantage point.
Long, painful Stone Age -type living, upon mainly Z. Flesh, but connected to an AbMS via a Ravine, found a few other new-in that world, Biomes, made Nether Portal.
Popped -out (knowing I was gonna make a roughly 10K E. and 15K N. Tunnel of Cobble, through the Nether - I justify it as "remelt" from the endless Lava Oceans, lol) in an average spot, Lava, Flowing nearby but-whatever, I continued to Protect the bus stop (so to speak).
Suddenly - in fact, after more than 2 min's. passed, so took me Completely by surprise (other than it being the Nether >.> ) - my "bus shelter" becomes inundated with more, Flowing -type Lava (which Flows as-fast as Water, there, so faster than I could walk-away), or rather, the River -effectively of it grows, Flowing me-sideways back into my Portal, aFire. I'm on Fire1! I freak out, unable to move-quickly when Lava-'ed -down, in it.. instead of continuing to go in just 1 direction... like the Portal, I try other directions, first.
Come back, a few items are sitting somehow on the edge of the Portal (none made it through) and the no-longer increasing, Flow of Lava (a.k.a. "lava flow," in R L). They were somehow-spared by that little bit of space between the Nether and the Dimensional Portal - and I am Trolled by some kind of Block Update, probably in the Nether "sky," up there by the ceiling, a Chunk Update - the World loses a little (non-Lava) luster for me, by that point.
No falls or Drops were taken in the making of this.. death.
Not the cliche "mining for diamonds"... Had full enchanted diamond gear, and I was actually leading some cows back to my place so I could breed them. Well, I was very oblivious of the surface lava pool behind me, and me and my cows, died.
More like: Fall in lava, get out of lava, die in small hole made by me, respawn, retrieve stuff.

Didn't read all the replies to the thread, but my most recent, and most costly death per lava was as follows: was worried someone was going to take all my valuable stuff, since I didn't have an ender chest yet, so I took THIRTEEN diamond blocks with me mining. Was branch mining as usual and come across a lava pool ahead of me. I drop a bucket of water one block behind me so I can be slowly propelled forward, continuing mining, while the water turns the lava into obsidian. Well, I mined a block in front of me, and I guess I went forward before the water updated and flowed into the same block and I fell into the lava, only the have to water turn the block to obsidian and then TRAP me under the obsidian block, i.e. in the pool of lava. I was so upset it took me a full 2-3 days to even want to attempt to get all those blocks back, but after a few weeks I'm back up to almost 9 blocks.
Moral of the story, always carry a water bucket, but don't get in too much of a hurry, lol.
I did this today. Was in the nether, fell in lava and died. All my enchanted diamond armour gone, sword, fortune pickaxe, now I’m stuck w iron again :((
I was once in a treasure bastion trying to find my way out, i was walking and there was one block lead to fall damage, and then i died and all my stuffs were burned, (i was near lava when i died)
And i have 28 diamonds, Iron tools, 23 Gold blocks, 8 chains, Iron Armor And the leggings are enchanted with unbreaking, boots, shield netherite ingot, netherite upgrade template, Raw iron, Iron Many Diamond Swords (i got those from the bastion)
Crossbow with piercing, Diamond Pickaxe, Shear, Cherry Logs, Cherry Planks, Flint And Steel, Obsidian, Gold Nuggets
(Yes I Still kinda remember my items especially the diamonds)
One time I was trying to find a lava lake for obsidian and I use chunkbase, then it was a bit underground and digged to the wrong coordinates, then I went into the right coordinates then mined straight down, I can totally approve that I could easily get a stupidity degree because I knew there was lava, man I had lotta iron and some diamonds.

While this never happened in any modded world I've played, it did happen quite a few times in my original vanilla world. I later went looking for the location of my last lava death, where I had gone into a cave and fell through a 1 block hole. I never did find the entrance to that cave, so I decided to just not look for it any more.
I've had items fall into lava quite a few times without dying, usually diamonds, but once in my original world, I had a faulty mouse on my old WindowsXP PC, and ended up throwing a clock into lava instead of a nearly depleted pickaxe.

Yeah Fire Resistance is a must, just like water breathing if you are playing survival in multiplayer.
