I got a diamond pick with efficiency V, unbreaking II
I decided I'd use it to do some crazy fast mining. I went down to level 12 and dug random tunnels. I was on one of those servers where you get xp for mining so by the time the pick was half up I was at level 40 or so with around 30 diamonds. I was digging so fast I didn't see the part in front of my break off into a ravine because it was so dark... I fell all the way down and barely missed the lava. It was a fall to my death and the diamonds, pick, and some other stuff had fallen in the lava. :/ I recovered only some food. D;

I made a full castle in Superflat mode and dug into the void to see if I could build under it and for some reason I panicked and didn't double-tap the space bar.. It re-spawned me no where near the castle :l I looked for like 45mins and never found it v----v
Just a few hours ago, I was playing on a server and going through the nether. There are pathways (about 5 blocks wide) over lava pools. So a ghast comes out of nowhere and shoots me off. I instantly log out and think of what I'm going to try to do. After about 2 minutes, I log back in and I'm safe on the platform! So I continue on... then accidentally jump off the pathway. I lost about 30 diamonds in total from that.
Had full diamond tools and armor (except for chestplate) and found a ravine. While I am digging STRAIGHT down, I always come across a hole and fall straight into the ravine and die from fall damage.
This one also happens to me a lot. I'm adventuring far away from my house mining, and find diamonds for the first time. I found one, and started digging for more. I then fell into a pool f lava with 30 diamonds and all my stuff.
I was mining in my cave (in my very first world I ever created) and I found some diamonds for the first time and so like eny other minecrafter that found diamond's for the first time I screamed *YES* and I was exploding with joy.Before I left the cave I though I was low on iron, so i decided to go mine a patch nearby. And BOOM i fell into a lava lake. After that moment I stoped playing minecraft for anout a month. And i learned
I made a mob killing pit around my house and put cactus in the bottom. I had no food points left and i was too lazy to harvest my wheat so i commited suicide in the cactus pit. Just one problem. I forgot to take all of my high grade armor and enchanted stuff off so most of it got destroyed by the cacti.
Had full diamond armor and diamond tools, and some other valuables. I was playing spleef with my shovel..... and I fell down into the pit of lava. I lost my tools, but one survived. I died again.

Was exploring a vast cavern / abandoned mine / ravine complex new to me at the time, freshly crafted enchanted diamond sword in hand. Had gone forward in a particularly dark passage to throw down a torch. As I was about to place it a cave spider jumped me unexpectedly. It startled me. My body spasmed. My hands left the keyboard. I quickly recovered and took a few ineffectual swipes at the beast. That's when the zombie appeared out of nowhere.
Unbeknownst to me due to darkness, I was standing on the lip of an underground ravine. The zombie's blow knocked me off the ledge. Down I went. SPLAT! To my surprise, considering the distance involved, I survived, much to worse for wear. What's more, I knew where I was! I had fallen atop the cobblestone roof I'd made long ago to protect my old Obsidian Generator down at lava/diamond level. I turned in place...to see a Creeper barreling down on me.
I went to whip out my trusty sword...except it wasn't in my inventory. The sickening truth hit me. When the spider startled me up above and my hands left the keyboard, I had repositioned them incorrectly. Instead of swinging at the spider, I had thrown my sword at it. My beautiful new enchanted diamond sword.
A quick look around confirmed that the sword had not followed me down. By this time the Creeper was on top of me. With no decent alternative weapon available I took off running. I hoped to make it to the nearby stairwell and lose the darn Creeper. Alas, it caught up with me before at the stairs. BLAM!
The upper portion of the stairwell was demolished. I had again miraculously survived, though by now my health was hanging by a thread.
Thankfully, that was the end of it. I removed myself from view by mobs perched on the ledges above and let myself heal. I briefly thought to ascend a column of dirt to retrieve my blade, but the Creeper chase had thrown off my sense of direction. I had no idea where in the vastness above I had fallen from. With heavy heart I repaired the stairway and returned to base.
You can bet I had reprogrammed the Toss/Drop key before my next play session.
First time I went to the Nether I didn't think and just ran out of the portal and fell right into lava. Luckily since I was sure I was going to get killed there I didn't bring any good items, the best thing I lost was an iron sword.
Prolly playing on a SMP server with friends, getting way ahead with my usual tactics, finally getting an awesome base set up only to dig directly down...right through my 1-block thick floor into a ravine ¬_¬; it doesn't end there...natural obsidian farm under it broke my fall and my legs and my arms and all my items spewed into the water...which flowed nicely into a lava pool =___=;;
Went back, used gravel to make a tower to dig down to the obsidian and get what little remained of my stuff....spider jumped on me from above, knocking me off, onto the obsidian again =____________=;;;;
Thus, I lost my advantage XD

One time i made a tree house at a jungle biome and by the time i didn't put a bed in yet and when i was finished building the roof a thunderstorm came in and lightning bolt came in so close to me that i backed up and fell all the way down.
It took me about a half-hour to find my way back and all of my stuff got de-spawned its more of a "CRAP!!" moment though

I was on my survival world and i had just managed to finish my sheep farm.
Thats when my wolf thought it would be cool to come and eat all my sheep >
That wolf got lock in a hole for a couple days.
