Created a Nether portal and went through it. So that Ghasts wouldn't have the chance to shoot it out I walked straight out. Turns out it spawned mid-air over an ocean of lava.
That's why either, you take flint and steel with you, so that if a ghast does shoot it out, you can just relight it. Or you go in without anything, bolt from the portal and try to get a grip on how to exit the portal so that such things don't happen for when you come back through with your equipment.
MY biggest and worst no moment is when in a SMP server I spent 29 diamonds and 2 iron and died from someone. I was like NOOOOOOOOOOOOOOOOOOOOOO FUUUUUU!!
I had about 50 of them over the course 15 minutes. All of them falling to my death in SkyGrid.

My worst "NOOOOOOOO" moment was when I made a HUGE redstone contraption that was an automatic wheat harvester, destroyed a block holding some water... * FOOSH * bye bye redstone...

Every day I build something with water and redstone in close proximity to each other. Every day I tear my liver out and smash it to bits on my desktop because something ALWAYS accidentally the water, or the creepers, into the redstone. (Usually me but sometimes it's truly unforeseen)
No one redstone-tastrophe is the worst NOOOOOO ever, but cumulatively over several weeks of building autofarms, grinders, rail intersections, etc. and watching dozens of repeaters and wires being washed down into ravines or blasted into thin air or whatevs, I'm ready for a tall, stiff drink.

When I first started playing minecraft, I didn't think you could break ice. I drowned with 50 iron ores, 9 diamonds, and a full set of iron armor. Diamond sword.
Went to the end with all my gear, diamond armor, tools the works. Spawned on a platform off the main island, like when it's floating ya know? So I turned and started making my pathway to the main island, and suddenly I'm flying through the air. Turn around to see that the dragon had rammed me. Flew straight into the void and lost everything that I had been working for on that world in one single go.
I was backing away from two creeper but got blown up by one that was around the corner in my mine. I swear that creeper was waiting for me to pass up that corner lol

Underground in a cave that was attached to a ravine.
Im digging and come to an opening that has water and lava pools it is about 20ish blocks down. So i jump and aim for the water. and miss and died....Not the worst part yet... I run all the way back to jump down and make it, i get all my stuff back, turn around and get hit with 2 arrows and launched into the lava pool. to make it out 3 blocks from the lava and die again.... then jump back down again and miss and die.... so much fail in less than 5 minutes
