Wearing full diamond armour, pushed into surface lava by chicken, no water bucket, died, the end that was a sad day, goodbye 24 diamonds
That sucks! Mine was when I was on top of a pillar of stone and dirt that was just jutting out above the ground about 60 blocks, and I saw an Enderman at the bottom, and decided to try to shoot it with my bow... it teleported right behind me, shoved me right off of the cliff, and died right after me. It was pretty ridiculous!
I was in a nether fortress, slaughtering zombie pigmen, and exploring for wither skeletons. At the time of my death, I was peering out of this sudden stop in the fortress passage, where the hallway randomly ended and dropped about thirty blocks into lava. I think: "It's a good thing I have my fire resist potion", and turn to walk away. JUST as a zombie pigman decides to take revenge for the slaughter of his kin, pushing me into a lava. Again, I thanked my forethought of drinking a fire resist potion, and start swimming to the side of the lava pool. Unfortunately, the lemming-like coding of the zombie pigman made him leap off the fortress after me. The last thing I saw was "goldendragon1597 was killed by a zombie pigman", and the hiss of my precious diamond sword incinerating in the lava.

My most embarrassing MC death(s) occurred not too terribly long ago. Bear in mind that...
I had recently updated from MC125 to MC142.
The HD texture pack I normally run had not yet updated to MC142. I was therefore running an unfamiliar pack, and not familiar with how various mobs were skinned.
I had not visited the Nether since updating.
I had read but misinterpreted the implementation of Nether mobs in the overworld.
After some days adventuring under MC142, several enchanted diamond armor pieces were in need of repair. I had been looking forward to this, wanting to try out the new anvil. I'd earmed fifty experience points, a record for me. Surely that would get the job done. To play it safe, I brought along some raw diamonds and a spare set of diamond boots.
My enchantment room and anvil are housed in a dedicated building located at one corner of my fortress. A ladder descends from that building to a passage that opens onto the plateau containing my Nether Portal.
There I was, standing at the anvil, about to begin repairs. Just prior to starting I glanced out the window toward the Portal plateau. To my great surprise two mobs stood there, one of which looked to be a skeleton.
I couldn't understand it. That plateau has been safe from mobs for ages. Yet there they were. I decided to kill them quickly for the experience, then resume repairs. The sun was up. I knew I had to rush before daylight claimed 'em.
I descended the ladder, ran to the plateau, and attacked the supposed skeleton(s). WHAM .... WHAM....WHAM....respawn screen! I was in utter shock. How in the world could a skeleton have killed me? It made no sense.
Expecting the sun to have downed my foe(s), I ran back to reclaim my stuff. It wasn't there. I went to the plateau edge and looked down. There it was, at ground level. "No biggie," I thought. "Not that far a drop. I'll grab my stuff and be on my merry way."
I jumped down and picked up everything in sight. Opened my inventory. Oh oh! Stuff missing, including my armor and sword. Just then I heard something approach. I existed inventory to see a mob barreling down on me, a mob in my enchanted diamond armor wielding my enchanted diamond sword. WHAM...respawn screen!
What the...????? This time I had the sense to read what had killed me. Zombie Pigman.
This began a whole string of deaths as I tried to reclaim my dropped gear then kill the Pigman for the rest. No way could I melee it, not with it wearing my enchanted gear. Without going into gory detail, I'll just say it was at least six more deaths later before I finally lured the creature into a position where I could bring it down safely via archery.
When all was said and done. I had not lost much of value. Those backup diamond boots were the worst of it. I did however, lose all fifty experience points. What's more, by this time all my reclaimed diamond armor was at the breaking point. You better believe I spent my next Minecraft session at an experience farm!
This fiasco came about because the article I read on Nether mobs in the overworld stated that they "pass through the portal" but did not mention pigman spawning near portals. My Portal, on its Nether end, is enclosed in a door-sealed structure. No mobs can reach it. I therefore assumed I'd see none in the overworld and never even considered that what I took for a skeleton might be something else.
Climbing up a waterfall that's almost as tall as my cave is high to get onto a high ledge to light it up a bit. Then heard my animals through the ground and saw coal onto the ceiling. Piller jumped a bit and made a bridge to get the coal. After that, I piller jumped again and mined up until I made a hole in the pig pen. I closed it up and started making my way down safely since I'm high up. Fell off and landed on the waterfall and the ledge, losing all but like one heart. Then I forgot to hold space and stuff to use the waterfall as a water slide down, and fell to my death. Never again I'll climb up that waterfall.
Back in the day, I went on a mining trip on SMP with a friend. By the end of it I'd collected 6 stacks of iron and about 1.5 stacks each of all other ores and a half stack of diamond. Then I found a vein of 12 diamonds (2 veins connected) and while mining it, dig straight down and fell into lava. My friend got really mad that I'd just wasted all my effort mining all that iron.
