I have actually! It was just yesterday when i was in the Nether Making a 1 by 1 by 1 cobblestone bridge to travel across the lava ocean when BAM! Hit by a Ghast that i did not see coming. Knocked straight into the lava and i couldn't get out for a few seconds and tried to run back to save my stuff, but it was too late and i lost all my diamond armor and tools. So when was the last time you died and how?
I came within half a heart last night. The teleport glitches nearly got me. I was boating back to my village 7 of my tamed wolves teleported to me while I was going upstream. As I got near the shore all seven teleported one after the other right onto me.

On topic: no, I have not. Any possible ways to die on my world are usually wiped out quickly.
I usually don't as I set up out posts and a rail system throughout everything so there's food and farms all over, but you kind of need to on an SMP, the last time I died a creeper blasted me into another creeper on the very bottom of the map while I was making a train station for the mine I was building
Actually I did die recently and for the first time in a very long time I died from a mob. I was in full iron armor (chest piece even had prot 2 on it) and fighting a zombie and skel at the entrance to a zombie spawner on a ledge by a long drop off down to lava level in a cave. Just finished the zombie off when I hear SSSSssss behind me. I swear the damn thing had a short fuse because I had no time to do anything before BLAM I'm dead. Lost my chest piece and boots but I think I was able to recover everything else of note.
Yes. I was working on a mountaintop tower in order to try and increase my storage space, and the game didn't recognize me hitting the sneak key somehow, and I was working on the side near the deadly fall. Thankfully, the mountain is right next to my house and the side I fell from is facing towards my house, so recovering my stuff was a breeze.
I died in my hardcore map played like 30 minecraft days and lit up a massive cave/ravine system and wanted TNT to connect it all together. Started my first piston mob trap and fell off the roof and died, hardcore, game over man!
I recently died to a huge magma cube. Didn't quite realize how much damage I was taking.
With my current projects, I usually die to drowning. And less recently, I died twice in a row by porting to the Nether, glitching, and falling into the void.

I died today because my computer started lagging... I fell to my death. Majority of the time I die from being careless rather than by the hands, or non-hands, of a mob.

Just the other day I died due to my stupidity. I have an xp grinder setup, and I hate having the loot just laying about so I have blocks moved out of the way to where I can get close enough to pick the stuff up. Unfortunately this also allows the mobs in the trap to see me. On that one occasion, I had 5 creepers in the trap with a few skeletons and I moved forward to get the items. I didn't move far enough back for the creepers to forget about me and while I was whacking away, BAMMM, death due to stupidity.

Lava usually gets me some way or another. The last time I died I'd found an undeground lava reservoir and I was looking around it for diamonds and a skele cam in from the tunnel behind me and shot me into the lava.

I was on a Minecraft TF2 server and well, I need improvement.
Those creepers on hard are very deadly now.

I tried it with terrible results. I gave my wife my glasses for a second, a creeper showed up and now my wife is pregnant.

was so terrible i yelled
The problem?
The explosion broke into a lava vein that burned me to death.
It was annoying to slog back to grab my stuff...
