Mine has ALWAYS been this; 1. Pickaxe 2. Shovel 3. Sword 4. Axe (Bow if I'm mining) 5. Bow (Axe if I'm not mining :P) 6. Torches 7. Misc 8. Misc 9. Misc Even in Terraria it's similar to this. I think it's like this because I made them in that order. Plus, I thought pickaxe would be the most important, followed by shovel. Sword is 3 because in a split second my middle finger can shoot up and press 3. Creepers really train your reflexes, especially for someone like me. Haha.

This was a little idea I had while starting X's Adventures In Minecraft over for my spare time today:
Everyone has a hotbar set-up that is unique. People prioritize things differently. With that in mind, I'm asking people to share their hotbar set-up to give new players some ideas, and maybe refine an old player's ways.
Here's mine, coming from Minecraft Alpha:
With this my offensive stuff is right next to each other, as well as emergency food. All my harvesting tools are next to each other, and I have materials for making bridges and stairways inside a cave. Torches are always 9, so I can switch to them when all is safe. Lastly, my utility helps if I may find lava by carrying a water bucket, or keeps me on the right track if I'm away from home. It also can hold a fishing rod for emergency food shortages or a hoe for farming.

So, first things first... I'm more of an on-the-fly kind of guy. Whatever can save clicks and time are things that make my MC-life easier as it is easier for me to figure the time it will take while playing in my head than move things around.
That said, I have a few placements that I like to follow but again... on-the-fly.
Exploring:
1) Sword and Torches need to be next to each other; pretty easy to figure out why on that one. Nothing like having a creeper jump down your throat when you light up a room.
2) Gravel stack has to be on the bar as it is useless for most crafting but falls when I find a ledge I need to fall down. Saves crafting another
pick trying to get down other ways as I rip through it with a shovel.
3) Fifth Element kinda thing on this one: Compass somewhere on there as I like to run towards light.
4) Pick tool, Shovel and other... usually an axe next to each other and the pick tool closest to the torch/sword.
5) One open as I want to know what and how many I am picking up... at least the first thing which once done get shifted to the inventory.
6) If I don't need anything else... I go Spartan. I leave the other open as well. A boat comes in handy on the seeds I usually use but not for sure.
What I carry is a different story altogether. ;-)
Building and Terraforming:
1) The standard Pick, Shovel, Axe orientation.
2) Crafting table for that quick item craft.
3) Torches
4) Sometimes these other items: Bed, Compass, Map, Boat.
5) Cobblestone and Dirt. Soon to be Stone and Brick but will change on-the-fly again on that one.
So, if I'm not exploring and just trying to exist and beat something... SWORD, TORCHES and food.

Building a straw made of stone and ladders since: March 2011. I is before E except after C unless it is weird.

I use random to display ores and diamonds while mining (it's inspiring XD) or for items that I need for a specific task (seeds while farming, map while exploring, etc.)

7: Item based on situation (Blocks if building etc.)
8: Whatever pops up into the slot

(Using this setup allows quick switching between the only 3 things needed while strip mining.)

6. Free slot, usually cobble or dirt.
7. Bread/Steak
8. Healing Potion II (For emergencies.)
