For me, it was raining on hardcore, so skeles, zombies, and spiders were all still threats. I walked out to go to my mine shaft when a skele popped up and got me down to a few hearts, then I saw a creeper, so I waited for my health to recharge and I headed out there. When I got to where the creeper was, he wasn't there, so I thought he despawned. I turned around to go home, but a familiar HISSSSSSSSSSSSSSSSSSSS appeared. I survived with 3 hearts left and got home.
I was on my friend's server and I got stuck under the void through a glitch. Trying to save myself, I went to the creative world through the /goto command and something weird happened. Instead of going to the spawn of the world, I ended up in an abandoned part of the map. Not only that but nothing would load outside the chunks around me. I was standing on a small chunk of nothingness with no way to return. Even relogging didn't do anything. To return, I had to get the admin to tp me.

Most frightening moment for me was just a few days ago. I was playing Minecraft while I had EVE minimized. In EVE I was transporting some stuff through a safe area, so I figured I could just do some cave clearing in Minecraft while the autopilot handled things. About 10 minutes into the cave, I was fighting back some zombies and spotted a spawner through a small opening. Oh great, a dungeon, I thought. I fought my way into the dungeon, slapped down a few torches, and started looting chests/mossy cobble.
Then an alarm sounded, indicating my shields in EVE had been brought below 25%. I panicked, minimizing minecraft and bringing up the EVE window. I was expecting the worst. I figured some pirates had found me and were doing a suicide run on my hauler.
Turned out to be some friends, messing with me. They knew I was autopiloting and thought it'd be a good laugh.
I was getting chased by a spider, a skeleton, and several endearment when I heard a hissing sound and came face-to-face with a creeper. Our faces were REALLY close, so close and someone might have thought I was kissing the creeper from a distance. The creeper exploded before I could move and I still survived though I wasn't wearing armor.
I'd have to say the first time I heard a slime. I had never heard one before, so I had no idea what was going on. The clicking kept getting louder and louder. For some reason my heart was pounding like crazy. Then I found the slime, and I realized I kind of overreacted.
