My biggest face palm and subsequently a drastic change in my fortress designs occured while I was finishing the storage room of my glass fortress version 1.0 (which was intended to get designed, then put onto my server, and included the bulk of the fortress residing inside a large underground pool of lava, airlocks and dispenser trap wall barriers and multiple routes that were difficult to bypass and usually resulted in death unless you knew them). It requires a bit of leading up to it to really feel the full impact of how bad it was. (sorry for the wall of text)
Up to this point, I was dedicated to doing this the hardcore way (no spawning items, no deaths) to ensure that it was possible to build this without "cheating", and I was going to make the schematics public if it worked out well.
I had run out of coal/charcoal for torches while building my cactus farm inside my glass fortress. I left the room partially complete with the sand and stone where it was figuring that the lava and torches through half the room would be enough light to stop spawns (mistake 1), and decided while I was burning more wood I would begin the process of moving all my stuff to the newly made material stronghold since I had to go get the cacti anyway (mistake 2). This stronghold was within a room that was fully surrounded by lava, had double layered walls, was guarded by an iron door air lock filled with water to kill any mobs that passed through, and had a trap on the final door that if you didn't hit the left hand switch then step on the pressure plate, it would fire both walls of dispensers inside the airlock (about 12 arrows each side). Not that easy to get into, and about 30 blocks down from the surface.
Pretty much only reasonable way in was through the mineshaft transport tunnel, then through the triple express elevator. (3 sheer drops in complete darkness, 2 end in traps, 1 ends in a water current that drags you through a tunnel as if you're going to drown, but then kicks you out into the main hallway just in time). It takes some time to get back down there...
While transporting my materials, I was passing my cactus farm, and unknown to me, a creeper spawned inside where I had run out of torches, AND I was dumb enough to not door the room (mistake 3). I had not loaded up the presure plate triggered dispensers on either side of the stronghold air lock since that was the next step of the item moving, and the water had not been put into the airlock yet due to the fact that I had misplaced the iron doors and had to craft a new set inside the storage room.
I was stashing into my stronghold before building the iron doors when from behind me I hear "hissss-BOOM". I shouted profanity as I died and jumped out of my chair startled completely silly by the suddenness of it. The creeper broke most of the chests in that room (about 9 stacks of iron, 4 stacks of gold, and about 20 diamonds in my inventory and chest total, plus a diamond pickaxe and shovel, most of my food, most of my wood, my bow, and a wad of other useful things) and it broke just enough of the ceiling to open 1 block for the lava to come in, and consequently flooded the room with lava.
I lost about 8 days worth of harvesting, mining, and exploring. As soon as the death screen came up, I had a feeling I knew what was going to happen next, so I watched to see if I could see lava. Sure enough, the sound of all the items burning up, followed by the screen filling with lava. I got up, threw the soda next to me across the room (irl mistake when the wife got home and it wasn't cleaned up.... >.>), went out to the living room, and did 32 waves of horde in gears of war 2 before I cooled off enough to come back.
So much effort into making the building well defended, and I go and let a creeper spawn inside of it because I had a derp derp moment that cost me over a week of my time. Looking back on it, it's frickin funny.
I thought durability of tools was affected by the time you spent mining with it, not the actual number of blocks they broke, so when I made my first stone pickaxe, I didn't want my wooden one any more, so I sat there for about 5 minutes constantly mining 1 block of cobble to near breaking point.
So I'm playing in SMP. I make my house in the village, and I figure "You know what would be cool? a Ring of Obsidian, then a ring of gold, then a ring of obsidian..." you get the idea. So I'm thinking "Ok, my walls are wooden. I don't want them to burn. Just to be safe, I'll chop away the first layer of wood around my house, and when I'm done, I'll put it all back!"
So, yes, fire skipped a block over by it's sparks and burned my house down.
It gets better. This was before fire was nerfed, so the fire just spreads everywhere. Even to my nice little porch and the tree that was in between my house and my neighbor's. Who's house also happens to be made of wood.
I'm trying to douse the fire from the inside of my house when someone in chat says "OH MY GOD, MY HOUSE IS ON FIRE!" I reply, because I'm inside and I don't know about that burning tree "No, that's my house on fire. I derped." And he yells back (in all caps rage) "NO, THREE HOUSES ARE ON FIRE!"
Yes, I know my maths well. You see, the neighbor's back yard had some sort of wooden monument, and it was enough to go to the back yard of the house behind his, burning his trees and his wooden house.
Thankfully, we where the ONLY three people who built our houses out of wood. ...and we're clustered together.
...I still say fire shouldn't of been nerfed like it has been. Fire is Deadly for a reason. But I can see why Notch did it.

Yeah, I agree, fire shouldn't have been nerfed. It's deadly in real life, and so it should be in minecraft. Anyway, if they nerf fire, why don't they nerf lava?

I was playing SMP with a friend of mine. We had gone down to a really deep cave I had found and were exploring when I see my friend mining diamond... only after the first 2 veins did he finally mention that he for some odd reason didn't have any diamond in his inventory, and after a quick conversation revealed he was trying to mine with wooden pickaxes, and had discovered numerous iron and a few other diamond deposits.

Yes he was new, but we had already mentioned to him several time the teirs of tools... seems he didn't get it.
I once spent a week building a boat highway between my 2 bases, then, 3 days later, 1.6 came and repaired the bug the whole thing was based on. -.-

Over 90 % of the human population is stupid (in my honest opinion). Don't expect too much from them when you post something that actually involves thought.
Worst facepalm for me was that I was JUST starting out in Survival Mode the day it was released and thought that Coal was mined at lava level due to a rumor floating around (never just rumors, find the facts.) I had no idea that charcoal was a creatable item so I had to keep reseting my world everytime the sun went down. I finally went to youtube and watched a Survival Tutorial. Man I felt stupid when I found out how easy it was to find coal.

I built 60 block high dirt towers so I could find myself on the map lol.
