One thing that i do from time to time is build a nether portal into a wall, then either put a door behind it or just a passageway. So, you can walk right through the portal and to your secret room. (i have mine boobytrapped)
I almost prefer to use blocks to hide my secrets, rather than lava... Water obviously doesn't work for a faux wall, because it's transparent =P With lava, I'm always worried it's gonna spill out and burninate my thatch-roof cottages (or me!).
The water-conveyed pressure plate switch idea is pretty slick though... I've tried an X-or gated door with hidden switches on both sides... worked on it for 2 hours or so, before I gave up due to the sheer complexity, and when I stopped to think about it, someone could just dig out the blocks to the sides of the door and get through. =/
I usually have a floor tile that covers a small chest, to hold all my valuables (gold, diamonds, obsidian). I only do that for SMP, as SSP, you don't really have to worry about people getting into your stuff =D
My floor "safe" is more of just a small chest covered by a floor tile... I've been using dirt blocks for my most recent SMP experience, as the grass will cover it back over, and it seems less likely that someone would think that anything is out of the ordinary. Also, the SMP server I've been playing in is a underground cave-style one, and there is only one layer on top of the bedrock where the town is located... Not an even layer of bedrock though, so you can find a pocket surrounded by bedrock on the sides to drop a chest in.
I also like using multiple nether gates, staggered with water gateways, as entrances.
Kinda noisy, but definitely keeps the riff raff out.
you can hide a room puting water in a hole, and one block deeper put a sign. Down there there'll be no water. i made it to hide a switch, it's better in the sea.
Or you can hide a room behind a waterfall,
Use the lava door trick. (hide a minecart behind a column of lava, right-click the minecart to jump in and get through. and hide it behind a lava fountain.
Try to make sure the minecart is parallel to the column of lava, so that it doesn't get pushed into the lava by accident.

Well the problem with the lava door is...you have no way out unless you destroy some blocks...and their is a chance of dieing a horrible death
You can make a secondary escape route, like an escape vent. or a hole in the ceiling you can drop down from. you could also use an iron door to exit but make it only one way. Either way getting out can be an issue. you could also position it behind an actual minecart so that when you need to exit, you can just click that minecart.
Also, i didn't come up with the idea so don't thank me, thank the guy who taught me it.
1) If you are using the standerd Texture Pack, then you can place a in a normal floor. This can hold up to two secret items. Just make sure they are not flammable.
2) Watch this video: Try taking out the torches from the sand. This will only work in a sandy room. When you press a button, sand falls down to reveal a secret room behind!
I usually place a door and then a painting over it, with a hidden switch somewhere in the room and a hidden reddust circuit running beneath the floor.
You could also just have a hole in the wall, and place the painting over it.
so whats the point of secret rooms? just for coolness and looks
Either that, or for storing rare items/tools (read: diamond) on a SMP map. If it's hidden well enough, nobody else will be able to steal your stuff, and you don't need to keep valuables on your person at all times.
I personally just have a secret room under my castle filled with a few chests. To get in, you have to go down a ladder, which is hidden by the cobblestone floor. (One specific block of cobble needs to be broken, and it leads to a ladder leading down into the room).

Make a long room. The far side of the room will be the secret room and the front side will be the non-secret room. Now go to the walls. Cover them in two different times of blocks in adjacent horizontal lines like this:
Make sure one block is wood and the other is a non-combustible block. Make the roof and floor non-combustible as well. Now cover a reasonable size of the room with a this combustible - non-combustible pattern. Now all you have to do is walk up to the far wall and light one of the wood lines on fire. Wait, walk through, and cover up the If you do it properly, the room will look inconspicuous and normal. If you want, you can put normal stone behind the wall so only one column will let you through. If a griefer or treasure hunter figures this out the burning column, he/she will be unable to get in unless they dig through the stone (which is unlikely). I will make a video explanation shortly and I will post it to this forum when it is uploaded.
I just have a room behind a waterfall with a nether portal, then behind the nether portal is another room. Pretty simple, and probably not that effective, but if you make sure no torches or anything are visible through the portal it looks like there's just a solid wall on the other side.
A simple secret room would be an attic only reachable by a chimney.
It's easy to discover if you look closely but no one usually does.

I have a pretty well hidden/secure vault in my main save
The edges of the room which leads to the vault are all wood. In one of the corners (nowhere near the safe) is a wooden pressure plate (which is hard to see because of the floor). The door to the safe shuts long before you can walk over to it.
The door to the vault is made of steel and hidden behind a painting.
The only way in is to find the pressure plate, throw a random item on it (to hold it down) then walk over to the painting and walk thru at the right spot.
It's not designed to be foolproof (You could dig thru the wall), but it is not easy to spot. Even if you see one piece of the puzzle. And even when you know where the door is and the pressure plate is, many people are not clever enough to think to throw an item on the plate to hold it down.
The "cutting board" on my kitchen counter is a wooden pressure plate. placing an item on that opens up a door behind a painting, which leads to a secret room. The secret room is actually a natural underground lake that is my bath house/hot tub, however in the deep section of the lake (it's naturally 3 deep in places) there is a hole done like this:

If you drop into it leads to a secret room with a nether portal. I haven't gotten things set up on the nether side of things yet, but another secret entrance that leads to another room is what needs to happen, methinks.

The extra hole at the bottem is for the lava so that it can have a spot to sit and not flow out at you

The powered redstone holds back the water. Flip a switch and the water flows, knocks out the torch. Sand falls and gives you access to room.
