I've gone around in circles for nearly 3 hours. I would have dug straight up by now but I only have 1 iron pick left and its in the red, and I'm on lava/diamond level.
What do you do when you're lost in a giant cave system? I'm thinking of suiciding even though I have a ton of resources. 3 hours is just too much time to be lost.
I've done this on a SMP server and came back with 22 full Iron Blocks. Although i asked them to tele me home.
If you running low on a pick, use the resources you obtained to make another pick.
i suggest digging straight up and find you way home from the surface.
(1)press the f key to see how far extacly you are from the top
(2) use your bare hands, unless you come across anything tougher then stone
(3) I know it's *****-ish but switch to peaceful mode, incase you hit a dark cavern .

Probably the best advice, or, make a small camp if you have some wood(always carry 10 logs or so with you) and make a furnace, cook some iron and make more picks.
Have you sealed off any passageways? You might accidentally seal yourself in a level if you're not careful. Go back and open up any that you've sealed off, borrow a torch, see where it goes. Oddly enough I've actually done this, and been trapped in a dead end system ( no outlet to surface) for a couple days.
Did you use a waterfall or river to get from one cavern to another at any time? Most people forget about water travel when they are ready to go back to the surface, and waste valuable hours running in circles when their route up/out is flowing placidly down the wall just behind them. I've also done this one, and only in frustration did I throw myself into the current and swim up. Bingo.
Do you have enough wood to make a chest? Toss your valuables in there, take a screenshot with F3 on so you can go back and find it someday, then head for the nearest lava and go for a swim. If you don't have wood enough for a chest, and you absolutely want out, find a well-lit place, take a screenshot with coords, and suffocate yourself with gravel or drown in a pool. Come back and dig straight down and retrieve your loot.
speaking of F3, do you know where you are in terms of the coordinates of where you started out? It doesn't help as much as you'd think but if you've tried everything else, it might at least keep you from continuing to wander further away from home. A compass lies to you, tells you to run towards the needle when in fact your ladder up might be sideways or behind you. F3 doesn't lie - wherever you are, you can screenie or write it down, return easily from the surface and recover exactly what you were about.
Lastly if you're on SMP you can ask the admin for a one-way ticket out or use the /give command to give you just enough wood to claw your way out.
In future, always use a system of marking tunnels you've been in and the direction you were traveling so you can retrace your steps. Redstone dust, torches only on one side, coloured wool, signs, even rails or mushrooms help various people avoid getting stuck.
edit; I never go underground without at least a stack of logs. I've been under for weeks at a time and the only truly limiting factor is if you run out of wood. Food you can find growing on the ground or in a pool. Weapons you can make if you have wood. Arrows - zombie feathers and gravel. And wood. Don't ever run out of wood!

I'd dig up like they say, except for one difference: Get back to the highest elevation you can before you start. Get as high as you can with the Iron Pick (even in the red it should get you a good distance) and pillar-jump as you go. And yes, set it to Peaceful. Getting blown up by a creep at this point would just be ********.
In such a case my advice would be to dig straight up.
Start digging upwards (only 2 blocks) alternating between blocks so that nothing falls on you and droping dirt or the cobble you dig out under you while jumping so as to keep going in a straight line up.
When your pic runs out just switch to your hand. An iron pic in the red should last you 10-20 cobble blocks, which is near enough to the surface to start digging manually.
Do make a backup of your current save should you try anything in case you die to a creeper or something.

if he had enough wood to make a chest he would have had plenty of wood to make sticks and a crafting table out of, which would then give him more than enough stone picks to get to the surface with a 1x2 staircase.
its likely if he makes a chest down there he might not be able to find his way back to it

(yes, i foud huge cave systems, and never got lost, once died because of wolf!)

I used the digging up with a spiral staircase suggestion and it worked out perfectly, even found another cave system to go back to haha
