I can only double tap W, and even then it only works half the time. Using an alt key specifically doesn't work. I've been told this is a bug, just one of many that Mojang hasn't gotten around to fixing properly (and I've tried three different keyboards--HP, GE and Microsoft--so it's not that). I suspect it's a compatibility issue with certain systems.
It annoys the heck out of me that this bug seems to only effect a few players, and I'm one of them.

The [ALT] Key doesn't work well for most MMOGs and Single-Player PC Games.
On my Computer System, "Sneak" is totally useless; except, for Flight Control. "Sneak" doesn't even allow my Avatar to be a Tunnel Rat (crawling through 1 Block x 1 Block Horizontal holes.
?What is the benefit of "Sprinting" anyways?
My Avatar can Outrun & Outswim everything while using Normal Movement; although, Spiders, and the "Speedy Gonzalezes" (Zombie Kids), can catch up to my Avatar; if, I am not careful and precise about my Movements over Hills and through Trees and through Water.
Boats move significantly faster than my Avatar; which, is good for Mapping the Oceanic Shorelines. Horses can move faster than my Avatar; but, Horses aren't as fast as Boats. I have yet to ride a Pig; so, I can't comment on Pig Riding Movement Rate.
I've always double-tapped. I didn't even know there was any other option. Though I do have the 'w' key mapped to one of my mouse's side buttons, so I often move forward and double-tap with my thumb on the mouse.

Pig Riding.. its as slow as normal movement lol.
Sneaking, this kind of hides your player tag (in most places) so other players can't find you easilly.
Sprinting, its good for getting places fast, escaping from skeletons / other mobs (if you sprint while jumping around them, they miss you a'lot)
(ps, something went wrong with the quote function on my end so I had to add the quote manually, so its a lil messy)
Three Rings for the Elven-kings under the sky,
Seven for the Dwarf-lords in their halls of stone,
Nine for Mortal Men doomed to die,
One for the Dark Lord on his dark throne
In the Land of Mordor where the Shadows lie. One Ring to rule them all, One Ring to find them, One Ring to bring them all, and in the darkness bind them,
In the Land of Mordor where the Shadows lie

Formerly known as ORabbit around these parts.

It is a Windows OS/Computer System issue.

lel Then what do you use for backwards?
You can in 1.7 (not sure about 1.6) the default key assigned for sprint is Left Ctrl.
