I have had the habit of quickly jumping on the block below the one that I just mined. So... I fall into...
Air,
Lava,
Creepers,
Chunk errors,
My own skylight(did I get the term right?)
Well, I seem to only play on a difficulty level besides peaceful online. So for me, I always teleport to someone to see what there doing. And there always on a 1x1 50 feet high dirt tower where I then plummet to my doom.
I also always die in dungeons. REPEATEDLY. They just always spawn for me...

Mostly I'm caught by monsters on the way home from my sand pit and then die just before I get through a door to safety. Close runner up is early morning creepers though.
Three ways mainly (in no particular order):
1. game locks up for a split second, hostile mob I was trying to fight kills me while I can't do anything or the lock-up causes me to be unable to stop moving, then I fall into lava.
2. Running into one or multiple skeletons at night with no bow or armor.

Right now I have this underground "cave" that has minerals.....and lava. I don't want to move or change the lava yet so I try to work around it as I make the cave bigger and hope to find more minerals there. But of course I get impatient and sometimes walk right into the lava.....and am too slow or dumb to get back to ground in time.
On the plus side. At least I know where my stuff fell when I die :/
For me it's all about falling, but not from cliffs or anything that results in an instant death.
I always jump down from little too high places, lose a few hearts, then it happens again and again till I'm dead.
I died TWICE from falling from a high structure. I was making a gravel farm (gravel falls onto torch -> gravel resource dropped, possibly flint), like this: (side view)

(this was right next to a high ledge)
and I climbed to the top and tried to fill it up from there. So, I carefully held shift and walked over the edge (you can't walk over edges in sneak) and placed a gravel block below me...

... let's just say I found out the hard way that when you sneak and place a falling block below you, you fall with that block.
Actually, it wasn't the fall that killed me, but the army of creepers waiting for me at the bottom.
As I said, I did this TWICE, because the first time I thought I had just let go of shift.

then a 2 block deep water pool on the bottom opposite to the ladders, however I recently learned it should be 3 blocks deep or else i die.
P.S. water is new technology to me, I used to only press forward then back and hope to land on the ladder before touchdown, which was coincidentally almost never
