The dispenser should be disabled from shooting items if there is a block in front of the output side. Just a small suggestion considering that most blocks don't work when blocked (note blocks, pistons with signs on it, etc...)

this may not be suited best for dispencers, but maybe droppers? I was making an automatic brewing farm and the best design involves levers controlling the output of materials using this concept
I disagree as well. How is it off-topic to bring up the kind of creative features that the existing feature brings up? The fact that dispensers fires straight into walls is very relevant for SEVERAL diferent traps involving for instance potion of harming and a wall connected to a preassure plate.
Everything reasonable there is to say on this topic has laready been brought up, thusly the only thing left to say is

Noteblocks work if other noteblocks are on top of them, hence other tones such as bass, they output different sounds (different instruments) when placed on different blocks or when placed next to them.
Wouldn't it be easier to have a block on a piston elsewhere from the dispenser against a comparator clock using a testfor @p[X,Y,Z,R] in a command block, to toggle it on/off when needed and via remote?
you can make floors and walls that give invisible damage to players.
You can already do that with the /effect command in a command block and again using a comparator clock with a testfor on a radius from the block you want the effect to occur at, it can even be a block deep in a wall or underground or anywhere so long as you write the command properly.
/effect @p 7 5 5 delivers 'Instant Damage' for 5 seconds at level 5, of course that would be instant death but the level can be reduced to a lower amount and time frame that suits the need and depending on whether or not the effect is set to trigger against a repeating clock or one that cycles one time allows controlling of the effect or when or how often it happens.
Aoccdrnig to a rscheearch at Cmabrigde Uinervtisy, it deosn't mttaer in waht oredr the ltteers in a wrod are, the olny iprmoatnt tihng is taht the frist and lsat ltteer be at the rghit pclae. No support.

I guess I'm not sure why you want this...
