There should be some kind of system that gives negative effects for not using a bed for an extended amount of time. How quickly it comes, and what the effects are would be depend on the difficulty setting. Don't flame me in the comments with things like "What if you can't find sheep?", there should be a 3-day grace period when you spawn, so that lack of sheep doesn't screw you over. However, I don't think anything damaging should be an effect. I think it should just be things like slowness or dizziness. Maybe extreme cases of fatigue could have a few hearts of damage, but nothing fatal.
No. That's too much. There's a lot of exploring and risks in the game, and we don't need to pile on this fatigue stuff. There's a lot to do in this game, and rushing back to a bed every x amount of days just to keep away this fatigue will end up being completely unfun and possibly game ruining for some. Though I do understand that you're putting more use to beds.
No support. This could take away the exploration of caves or land for a long time. This is too realistic for Minecraft.
I guess you could also have certain things that also affect fatigue, such as light level, items, or biome? Plus, it isn't that hard to take along an extra bed.
