The smoke block would be a dark block that resembles a black wool block; It's purpose is purely aesthetic. Much like a burning piece of netherak the smoke block would produce smoke particles. The difference is that they would move to a higher elevation before fading and the smoke block itself does not actually burn or produce fire. This allows it to be placed at varying heights inside objects to produce a chimney effect.
As far as crafting patterns are concerned I was thinking a 3x3 block of coal, possibly the middle block could be something different like a flint and tinder.
No reason it can't be crafted in survival is there? Once your fortress gets awesome looking maybe it needs to billow some smoke so the villagers know whats up.

I thought it would be about fire releasing smoke blocks that move up then disappear. That would be very cool. I think this belongs in creative mode as a special effect.
make a smoke projector, that projects an air-type block, with a hazy animated texture, along with that, smoke particles would float out of both blocks on activation/projection.

As you can see fire produces smoke particles that mostly disappear about 1-2 blocks above the fire, and they're pretty sparse by then any way. The smoke block would produce smoke particles that last a bit longer and remain thicker together for about 3-4 blocks. This can help make chimney or smokestack effects more pronounced. The smoke block itself would simply be a dark colored block that emits the smoke particles.

On second thought, I think this would be good to have in creative mode, but I still don't think it fits in with survival.
