It's a really simple suggestion but I do not know if it's been done before, but it's a simple way to prevent griefing amongst players in creative mode.
Basically, the default creative mode in LAN and servers contains all of the abilities creative would normally have minus the lava bucket and TNT, the two most favored items by griefers. Gunpowder would also be blocked to prevent just crafting TNT.They could still destroy blocks one-by-one, but that takes so long that most people wouldn't bother with it. Of course, the item values of these things would be blocked on the in-game cheats.
This shouldn't be a much of a bother to someone who just wants to build or use creative mode, but discourages bad behavior. To receive full privileges a person would just have to be approved by the moderator, not just another player with cheats. It isn't full-proof, but should help.
This wouldn't really fix the griefing problem. Griefing is being annoying in any method, not just breaking things. For example, if I have access to Creative, but not lava, gunpowder, and TNT, but I could still light everything on fire with Flint and Steel, flood someone's house, fill it with gravel, ruin Redstone circuits, place Bedrock everywhere, hell place ANY block everywhere, and about a thousand other things.
So in short, restricting Creative mode a tiny bit wouldn't do anything to deter griefing. The only way to deter it is by only playing with people you know, or at least knowing how to run a server and using a whitelist to keep unwelcome people blocked.

Well, Badprenup, you pretty much summed it all up. In fact, you don't even need to place or destroy blocks to grief someone. Simply standing in their way or pushing them around can be just as annoying.
Breaking blocks one-by-one doesn't take that long. Most griefers tend to use hacked clients, where they can find the required diamonds (which also give EXP) to make a efficiency pickaxe, which works like a chainsaw to just about any resource.
