We all know about the Wither potion effect from Wither Skeletons and the Wither itself. Well, apparently one of the unused potions is a Potion of Decay, which gives you the Wither effect. So why not make it brewable?
Like the other negative potions, it's quite useless as a drinkable, but can be thrown, and the duration can be increased with Redstone, OR it can be made into Wither II with Glowstone Dust. They can also be turned into more useful Splash potions, throwable at enemies in PvP, but a bad shot can, as always, make it hit you too if not only you.
Considering that the Wither attacks any mob, the splash potion would damage any mob, including all undead hostile mobs except Herobrine.
Variation 1:
I made it two variations because I couldn't decide.
Killing Wither Skeletons would make them drop Wither Bones instead of regular bones, in addition to Coal. The Wither would also drop 2-3 of these. These would be the ID of Bones, but with a different damage value. These bones would be crafted into Wither Bone Meal, also with the same ID as the white variant, and with a different damage value. The Wither Bone Meal would then be brewable with Awkward/Mundane potions into the Potion of Decay. (see above)
Wither Bones would poison wolves, and Wither Bone Meal would destroy any plants or crops or anything, excluding trees, in the area of where you use it on the ground, causing them to drop 99% of the time. The radius would be the same of regular Bone Meal. It could also possibly be used to insta-grow Nether Wart. Using it on the Wart would not cause nearby plants to be destroyed.
Variation 2:
This one might not be as good, but I tried not to make it OP. It leaves the Wither boss completely out.
The Wither Skeletons would have a 50% chance to drop one three-use-left Stone Sword, with the new, non-enchantable Wither Aspect I enchantment on it, which will has a 2/3 chance of giving a Wither effect to whatever it hits. This can be brewed with an Awkward/Mundane potion to make the Potion of Decay. (see above)
In order to avoid the potion being brewable from a normal Stone Sword, this sword would have an ID of 272:1, or 401 or whatever the next free stand-alone value is. The sword would not be craftable, and as stated above, would only be dropped as a rare drop of Wither Skeletons, always enchanted. If you use /give to get it, the sword would be unenchanted, and act like a normal one. The Wither Aspect I enchantment can't be added to it legitimately, nor can it be added to anything else.
Hardcore PvP servers could possibly add a plugin that enables this enchantment legitimately on any sword if they want some deadly PvP, but this would not be possible in vanilla to avoid griefers or simply yourself being OP in singleplayer.
Sandpaper:
Much less useful than the Wither potion, but useful for some neat decoration.
Dig Sand. Craft Paper. Craft them together shapelessly. You now have one unstackable sandpaper!
But what would it actually do? If you right-clicked anywhere on the sides of a Wood log with Sandpaper in hand, smash particles from the side would burst off on all sides, and the block would turn into rotation-maintaining Smooth Wood blocks, much like Dirt turning into Farmland from a hoe. A new sound would be added too.
Smooth Wood:
This Smooth would wood appear as a slightly gritty semi-smooth surface similar to Smooth Sandstone, with the color tint of the wood type. It can be chopped with an axe, or anything, and would be collectable, placeable and rotatable as the same block.
They would also have their own block ID, with damage values for the different wood types.They would of course be flammable, but only as much as Planks. They would also last in a Furnace for as long as Planks, and couldn't be smelted into Charcoal.
The texture might be different, but it would be a nice rotatable building block. Slabs, anyone?

Sandpaper sounds like an interesting idea, but in the recipe you should add slimeballs since there much more common now and it would make sense for them to be used as glue to help the sand stick to the paper.

Maybe. But you can make a whole Mushroom Stew with two Mushrooms and a Bowl. No Water Bucket. The not-extreme, but not-small lack of realism in Minecraft is what makes it fun.
