Whenever you are attacked by a mob, there is a 10% of contracting a diesease. However, different mobs will have different diseases. In order to rid of your diesease you will need to consume any potion of restoration or healing.
Dieseases:
Zombification: Melee damage is halved.
Mob: Zombie, Zombie Pigman
Tetanus: Sprinting speed is lowered significantly.
Mob: Skeleton
Fever: Health and Hunger restoration is halved
Mob: Silverfish, Spider
Enderosis: Breaking block speed is quartered
Mob: Enderman, Enderdragon
Thermal Burn: Extra damage while on fire
Mob: Blaze, Magma Cube
Slimes have the ability to break your legs which doubles fall damage
I haven't decided for creepers and ghosts yet, but I'm open for suggestions!

Oh yeah, and we should also add a breathing bar, where you hold 'b' to breathe in and if you don't breathe for long enough you die. And if you go into water in an ice biome for more than a few seconds you die of either cardiac arrest or hypothermia. And you have to constantly drink water or you die. And every time you get injured you have to craft a splint or a cast and let it heal for a few months.

Seriously though, there's no point in adding a ton of useless problems for no reason other than 'realism'.
The only injuries you should get in game are those sustained from sheer stupidity when trying to feather fall off a cliff... and I am not just talking about any cliff...
And if people don't like it, then ask jeb to remove the hunger bar.
Your logic is like spreading jam on your face to help you play soccer better. Rethink that; how are play-limiting conditions remotely related to the hunger bar? Realism? Realism isn't an issue. The hunger bar is a challenge so that you can't insta-heal in the middle of a battle, PvP or otherwise, plus you have to actually work your farms to get a surplus of food, instead of letting them grow and being extra cautious in your mine. Realism has nothing to do with it.
Having mobs basically poison (for simplicity's sake, let's just replace everything you wrote with "poison", because that's essentially what it is, regardless of the tiny details) you upon contact (10% may seem like a small number, but it's actually about one out of every ten mobs you'll fight will poison you) does nothing to increase difficulty whatsoever, it just makes players die roughly 10% more often.
No, "hard/hardcore mode only" is not an excuse, nor is it a way to let it be added. Whether it's on one mode or the other makes no difference in how pointless it is.
