Dinnerbone tweeted about suggestions for this, and I want to put my two cents in.
1: Smaller ore veins
2: Less tool/armor/weapon durability
3: Higher mob spawn rates/caps
4: Weaker weapons
5: Mobs not burning in the day until noon
6: More abundant and larger lava pools
7: Longer block breaking times.
8: Max enchant level raised back to 50
9: More abundant dungeons (With less in their chests)
10: Poison able to kill you
11: Lower mob resource drop rates
12: All mobs being able to sense you through walls
13: Endermen being given the ability to pick up any block that pistons can push (Including ones that are dropped as items when pushed)
14: Less experience dropped by mobs
15: The scrapped "Mobs can spawn at higher light levels at lower depths" idea
16: More powerful Ghast explosions

Not to be rude, but these don't really make the game harder so much as annoying...it doesn't require a lot of changed gameplay style for the players, they can basically do the same thing as they've always done. Perhaps the only one worth considering is 15, since that would actually force the players to try to light up caves a lot more and think twice before venturing into the depths.
Personally, I prefer mine more, because it adds things that would force the player to actually think more about what they are doing and add truly greater difficulty to the game, rather than random nuisances.
some of these would only be applicable at world generation, which isn't really what hard difficulty is... but i agree some of these are just annoyances. i do like 3, 10, 11, 12, 14 and 16 though.

Curse PremiumPersonally, I prefer mine more, because it adds things that would force the player to actually think more about what they are doing and add truly greater difficulty to the game, rather than random nuisances.
