It is very annoying to have new adventure maps tell you that you need to keep switching gamemode, and since most adventure maps only give you these items to help you move forward anyway, I think they should be the only ones implicated until there are better ideas for it of course.
When it comes to having to switch game modes all the time, I totally disagree, that's just lack of innovation. Using pistons you can break any of the things you've mentioned by button press. If you're a good mapmaker, that should be enough. The only things I believe should be placeable/removable is torches. Plus, I don't think you should be able to access the dispenser inventory either. If you're trying to make traps, it becomes easy loot if you manage to avoid them.
I think the best course of action though, is as is said, for each map there should be a spec that specifies what can be interacted with.
Example: Buttons, Levers, Redstone torches, Pressure Pads, (Maybe Pistons) and tripwirehooks.
