In the past, boats break when you ram them into lilypads. In recent snapshots, lilies break when you ram boats into them. My suggestion is simple - make lilypads entities! Instead of treating the pads as rock hard chunks, they would be able to be pushed away. If you have trouble imagining that, think of it as pushing other boats aside instead. It would make things MUCH more fluid.
But then it might be annoying if a player tries to stand on it but instead pushes it away.
If it only worked when boats hit them, and no other moving object...
I don't think they should be entities all the time. Maybe it could work like the Dragon Egg, but go when you hit a boat into it instead of try to mine it (or maybe players could push lily pads around, which would be AWESOME), and (obviously) have a teleporting distance of only 1 block.
With this in mind, and boats being dropped as full boats, we could make hockey in Minecraft using a lily pad for a puck (though, obviously not if my teleporting idea was implemented instead of yours).
But then it might be annoying if a player tries to stand on it but instead pushes it away.

What about all of the current lilypads? Will they all automatically turn into these Entity Lilypads when placed? How would they react when attempting to place and destroy? Would they still have a hitbox?

And when you stand on them you will wobble up and down? And it will sink a litle bit if you jump on it?
I like it. Make the lily pads not just float around and be able to float away, real life lily pads are atatched to the botton by a wine. So make them only move in a small area of that, meaby like 2-1 block radius from set.
Perhaps they would pop back, but will break if pushed too far?

Sounds good. Do entities cause any performance drops? That could be a potential issues. If this is the case, what about having boats more or less pretend the lily pads don't exist?
