I am aware that this has been suggested before several times, and in several different versions. To my knowledge, it has not been suggested in this way. Please notify me if I am incorrect, and which parts I should change. I am open to all feedback, both negative and positive, if it is justified and not just posting "No".
With that put of the way, on to the actual suggestion!
Thought Crystals
Occasionally in dungeons chests, and as a rare drop from an endermen, you will find a "Thought Crystal" (Hereby referred to as TC). A TC looks like a small sliver off of an emerald with a slightly lighter tint. Please note that I am not suggesting that TCs are craftable from emeralds, only that they look like pieces of them. by themselves, if you hold a TC and sleep, then there is a one percent chance of going into the dream world. If you want to improve your chances, you need to put a TC in the center of a crafting table, glowstone blocks in the corners, and lapis lazulis (Not blocks) in the sides. This will give you the enhanced thought crystal (ETCs), which gives a five percent chance of going into the dream world. Combining four of these ETC to get a refined thought crystal (RTC). An RTC will give you a twenty percent chance of entering the dream world. If you put five RTCs in an X shape, and put dream powder (Explained later) in the rest of the slots, you will get a perfected dream crystal (PTC). The PTC will give you a one-hundred percent chance of teleporting to the dream world.
Dream World
Once you get taken to the dream world, you will see an erratic world generator, that spawns chunks far above the rest, caves that go straight down to bedrock, random 1x1 holes into the void, and similar strange world features. All of the blocks and mobs have a light green glow coming off of them, giving it a surreal feeling. Every time you enter the world, it will be randomly generated again and the old dream world is deleted to save space. When you enter it, only your hot bar and armor will be taken with you. There will be several new mobs and items, as well as old ones. First is a Dream Wraith. It cannot spawn within 30 blocks of another Dream Wraith. In any form of light, they become slightly larger (1 block more in each direction) and turn a slightly lighter color. It looks like a black cloud in a 2x3 size. it does 1 heart of damage in melee, but if the player is outside of 5 blocks, he will fire a small blue ball that does 1/2 a heart and won't knock you back, however it will turn you in a random direction. They have 5 hearts of health, making them easy to kill. They drop confusion orbs and dream powder. Dream powder is used in a number of recipes, such as the PTC. and the confusion orbs can be thrown at an enemy, making them attack all other creatures for 2 minutes. They will still attack you, but prioritize other creatures. If they attack a wolf, the wolf will attack it until one of them is dead, and if the wolf survives, it will become docile again. if you hit the confused mob, then the effect goes away. Next, we have the Shadow Traveler. This monster leaked into the dream world from the Nightmare World, a place of darkness. While not as powerful here, these travelers are formidable still. They can teleport from any area at a light level of 5 or lower to any other area with a light level of 5 or lower. They have a melee attack of 3 hearts and have no ranged, They appear as a man floating 1/2 a block above the ground in a black cloak with a hood and glowing red eyes. they have ten hearts, making them able to take a good amount more hits than the dream wraith. They drop Warp Crystals on death. These warp crystals, in a more contained form. teleport you between the dream world and the nightmare world. To exit the dream world, you must wait for the day to end.
Nightmare World
The polar opposite of the dream world, the nightmare world is a labyrinth of evil and destruction. To reach it, you must make a Stabilized Warp Crystal. You need to get 8 Warp crystals and surround a confusion orb with them. This will give you a Warp core, a key part in the creation of a stabilized warp crystal. You can right click with a warp core to absorb everything in an area the same size as a TNT explosion. This is saved and you can right click again with a warp core to place it again. Put a warp core in the center of a 3x3 crafting table and put a diamond above it, a lapis lazuli block below it, and dream powder in the rest of the slots. This will give you a stabilized warp crystal. In the overworld, nether, and end, its useless, but in the dream world, right clicking will take you to the nightmare world. It generates similarly to the dream world, and every time you travel to it, there's a 25% chance of going to the same nightmare world. The stabilized warp crystal can take you back to the dream world as well if you right click with it. The stabilized warp crystal only has 50 uses, allowing for you to travel to and from the nightmare world 25 times. Shadow reapers appear here, generally more powerful versions of the shadow travelers. They have 20 hearts, and do 5 hearts if damage, with a ranged attack of 10 blocks at 1 heart of damage. They drop 3-5 evil shards, which are used to created corruptors and compressed wickedness. This Corruptor is a ranged weapon that uses Compressed Wickedness for ammo. It will cause every stat (Including drop rate!) to double, and go into a frenzy, killing everything in sight for two minutes. If they touch water, they will lose this effect. If you use this on something with no attack (such as a pig), it will get an attack of 1/2 a heart. The corruptor is created with a row of evil shards on top, a confusion orb in the center, dream powder on either side of the confusion orb, and an ethereal handle, made with a stick in the middle, a diamond above and below the stick, lapis lazuli on the sides, and dream dust in the corners, and iron in the other two slots. Compressed wickedness is made with 4 evil shards in a 2x2 square for 2 of them.
This suggestion is still a work in progress, but feed back is encouraged.
The Nightmare World is completely optional, if that's what you mean. It isn't random chance, you just have to build a stabilized warp crystal and use it in the dream world. If you mean the dream wraith and shadow travelers, they're pretty rare and this was made to be more like an actual dream: A strange version of our lives as we perceive them, with dark things seeping in from the darker parts of our mind.

It's not really something that fits with the current theme of minecraft. IMO new dimensions should be based on areas that already occur in the overworld, but on a larger scale and with different mobs (nether resembles caves/underground etc). A dream world, or for that matter; any world that doesn't get saved when you leave, is a good idea, but it seems more at home in a mod than vanilla minecraft.
The way I could picture dreams working would be as short minigames that take place when you fall asleep while a jukebox is playing. These dream minigames would allow you to earn EXP if you play well, but it will end either when a time limit has passed or if a certain "failure" criteria is met. Each Music Disc causes a different Dream, each with its own goals, theme, and mechanics.
I'll admit it myself, this actually probably would fit a mod better. So, does anyone have any new ideas for this, other than their own version of the dreams? Also, consider the topic done, I'm probably not going to write anymore.
