I suggest a new difficulty, one that can be payed in survival or even in hardcore; called "Unforgiving". Each of these two features could be made optional and configurable in the server files.
Here are the differences, as compared to hard difficulty.
Limited Health Regeneration
-health will only heal up to 2,4,6,8, or 10 hearts. So if your health is half a heart, it will only heal up to 2 hearts. If you have 2.5 hearts, it will heal up to 4
-your health will only heal after you stop moving for 5 seconds, and are in a bright area (won't heal in darkness)
-the only way to heal more of your health is with a golden apple or health potion
Claustrophobia
-your character fears small places
-if your player is in a small area (determined by percentage of surrounding air blocks within a ?x? radius of player) and the panic sets in, then he will very rapidly lose hunger points
-this makes it impractical to hide in tiny holes and dig tiny tunnels
-UPDATE EDIT: instead of loosing hunger, it should instead show up those "air bubbles" that you see underwater. These "panic bubbles" (as I will now call them) would represent your panic level. They would drain if you are in a closed space, and if they get to 0 then your hunger bar drops very fast. This way the player would be aware of when they are experiencing claustrophobia, and will have a chance to leave the area before ill effects take hold. Bubble idea by empyreanunseen

I like your Claustrophobia idea. Maybe if the player is isolated in a very small space, those air bubbles that are done when swimming could pop up. After all, no space = no oxygen.
