I would really like to see very clean strongholds above ground. It could be filled with Villagers wearing Iron Armour, signifying that they are a much more advanced clan/country and are in the medieval stage.
These peasants and knights would trade with you as well, but they would give much better and rarer items early on in the trading process. However, they're still ripoff artists, just like the Villagers.
The structure of these strong holds would be different from the underground.
These aboveground strongholds will have all rooms on the ground or stacked in a way that doesn't give Newton the middle finger. There will be a surrounding wall, and on the outside a moat.
Generated Structures
Cottage > A basic stone house with one floor.
Inn > A stone brick house with two floors and a bed.
Bridge > Connects upper floors of separate buildings.
Landplots > A large field of empty but tilled and hydrated soil.
Animal Farms > A small Wood fence enclosure containing Chickens.
Ziggurat > A large tower in the city corner with the most importance.
Flag > A simple Fence and wool flag placed on corners.
Portcullis > A fenced doorway, connected by a bridge to the moat.
Market > A large, one floor building with 8 fenced secions in one hall.
These generated structures are placed randomly inside the walls of the Stronghold.
Villager types will spawn in their respective profession buildings.

Read it the 2nd time and I THINK I know what you mean by that...... doors being 4 blocks off the ground, and ****ed up buildings?
I think that some sort of enderman fits better to strongholds because of the portal. maybe they have silverfish for pets?
