Now, we all laugh when other people get YASD'd. Well these suggestions are to keep the lol's coming.
Torches burn flammable material they are attached to after a while. (TNT, BOOM! Wood, etc.)
Falling from a damaging height onto glass shatters it!
Monsters are able to smash glass. (arrows too.)
Plate armor is less effective against arrows, chainmail required. (makes you plan out your trips more.)
Fire monsters that spawn from underground lava!
Hunger/thirst meter?
Berries, along with poisonous berries! (Seriously, I lol'd after a while when I figured out red-white mushrooms were poisonous.)
Were-creatures, look like regular creatures, at night, they transform into horrific beasts! (maybe on the harder themes.)
Babies.

For the fall-smashy one... Perhaps, as a player gains momentum, they could gain smashy power? For example, falling from two stories up might go fast enough to smash through glass, 4 or 5 stories would be dirt, and falling from the top of the skybox would leave you a few layers into the rock.

Plate armor is curved to defect arrows, defects thrusts the same way, and is quite solid enough to absorb slashes.
The rings in chainmail help to balance the power of a blow evenly, but arrows hit a much smaller area.

Poison berries give you a bad trip as well! (Anaglyphish mode?)
