Firstly, 9 Enderpearls can be crafted into an Enderblock.

When right clicked, the emergency teleporter will teleport you to the nearest teleportation block. However, it will be destroyed in the process and leave you with one heart. This makes it useful for escaping certain death, but at a cost. It can also be crafted into a deluxe e teleporter, which will allow it to not be destroyed upon use.

Now, there is one more block in this suggestion, the teleport link.

This block is used as a permanent teleportation link. When a set of 2 are crafted, they are given a unique value, linking them together. When both are placed, standing on one will teleport you to the other. This comes at quite a cost though, as a silk touch pickaxe is required for the Diamond Ore.
I don't think this is needed. Yes, mods, maybe, but in vanilla it just seems very lazy when people are going to travel.
It really depends. the Link block would be designed more for traveling extremely long distances, where traveling normally isn't practical. This is would be useful for strongholds, as having a teleporter to your main base (which is most likely far away near the spawn) from a secondary outpost near the stronghold would save lots of time on travel. Another good use for this is with larger servers. instead of traveling obscene distences to free land, you can teleport directly there.
Not very, well, 'Minecraft-y". But in a different case it is a good idea. This would be useful for Capture the flag servers, or ones that have games. Just so you can get there quicker. Although, Bukkit. /Warp pacman

It needs to be balanced. Saving your life (and diamonds) from the lava you just fell in really needs to cost something, and a reusable method even more. The Link blocks need to also cost a lot, as the alliterative is to build massive railways with thousands of iron and hundreds of gold rails.
