I think as far as nighttime in minecraft goes, if the game is set to hard, then you should not be able to go out with no armor and a stone sword and survive. There needs to be an element of terror as far as going out during the nighttime. I think the difficulties should ramp the monsters up a significant amount more than they do now.

Agreed, More enemy damage on every diffulculty and also higher diffaculty levels

Difficulty in any game comes down to good AI for the mobs. More health with be more difficult, sure, but it would just take you a little longer to kill them.
What we really need is either:
More mobs, with different abilities
Mobs gain added abilities while in a more difficult mode
Mob AI needs to make them more aware of their surroundings
Now, I'm not entirely sure how difficult it is to improve mob AI, but it's pretty simple to avoid damage right now if you aren't surprised. A skeleton spots you at range? Just make a zig zag pattern towards them and dodge the arrows, then a few whacks with your sword and they are gone.
Now it's still easy to get taken down if you are overwhelmed by a lot of mobs, but by themselves, they are pretty much no match, even if you have no armor. Add in armor, and it's ridiculously easy to kill them.

Yeah, this is the problem that plagues most games, bad AI. Once you get used to how they fight, you, as a human, can evolve and change your tactics to counter theirs. Problem is, they can never "learn" from you. So increasing difficulty essentially ends up with increasing health, thus extending the battle, as opposed to actually making it more difficult. Long fights don't necessarily mean difficult fights.
