We have the standard modes available from the start: Peaceful, Easy, Normal and Hard. But people should be able to play and work their way up the difficulty ranks, and have an actual incentive and a goal to play each one. Consider the following:
Greenhorn: Score 1000 points on Easy. Must begin as Easy, and not change settings. Mob enhancements in this mode:
Zombies and Skeletons will have a chance of wearing leather armor.
50% chance passive mobs will aggro on attack
All mobs have a 10% less chance of dropping items, 0% with looting enchantment
Warrior: Score 2500 points on Normal. Must begin as Normal, and not change settings. Mob enhancements in this mode:
Zombies, Skeletons and Creepers have a chance of wearing iron armor, although will more likely wear leather armor.
Swamp water is collectible as is, but will poison mushrooms near the water. Has a 50% chance of yielding negative effect potions
Red mushrooms with blue spots, and brown mushrooms with a lightish blue stem are poisonous. They will grow to be poisoned huge mushrooms, yielding poisoned mushrooms when destroyed.
All passive mobs will aggro on attack, but will not gang up on you if you fight them, Bulls/Cows will only headbutt/bite you.
All mobs have a 20% less chance of dropping items, 10% with looting enchantment
Veteran: Score 5000 points on Hard. Must begin as Hard, and not change settings. Mob enhancements in this mode:
Zombies, Skeletons and Creepers will wear iron armor.
Skeletons will always fire flaming arrows.
Ghasts in the Nether will fire projectiles with a blue flame, noting higher fire damage (As blue flames are hotter than red or yellow flames), and will have a brighter light accordingly.
Blazes will also have a blue flame surrounding them, with higher contact damage. Touching a Blaze will engulf you in a blue flame, which does double damage, so fire resistance potions and enhancements are necessary.
Swamp water is collectible as is, but will poison you when drink it, and will always yield negative effect potions (Poisoned water).
Poisoned mushrooms will be more common than regular mushrooms, as poisonous swamp water has a larger poison radius.
Villagers will attack you if you attack them. Villagers, when provoked, will leave their homes and provoke you. Spawned villager homes will aggro villagers when being destroyed.
Other passive mobs will attack you if you attack them. Bulls will headbutt you, and drag you a fair ways causing both direct and indirect damage, before stopping, tossing you off, and hitting you again.
Sheep will bite, and gang up on you
Chickens will peck/bite, and gang up on you
Pigs will bite and gang up on you.
All mobs will have a 30% less chance of dropping items, 15% with looting enchantment
Hardcore: Score 5000 points in Veteran

Begin in The End, with the same land generation as in Dream world! To escape, find an ender portal, that will teleport you a random stronghold in the overworld, in the only stronghold that exists there. Mobs switch locations, with a lava lake and Nether mobs in the overworld, and a water ocean with overworld mobs in the nether. Grow food with lava instead of water, and grow Nether Wart with Water.
The point of a sandbox game, and creative especially, is that players can do anything they want no matter what. This would ruin the sandbox feel and make many people angry, as they can't have access to some of these features at all.

Do you feel angry you can't just spawn obsidian in Survival, that you need to mine four iron, collect one Flint, 8 stone (for cobblestone, 1 coal, sand to make a mold, a nearby lava source, a nearby water source, and four trips, to make an infinite water pool in a 2x2 pit of sand, 16 trips to get the lava, to make a sand staircase to make the lava spring block spawn higher up, so you can make a obsidian portal, to light up with flint and steel, to go into the Nether?
The last three suggestions for "Difficulties" don't need to be locked off for creative mode. In fact, since there is a switch for modes, Hardcore could be a relegated to a difficulty, and creative mode can have only Normal, Dream world and Bizzaro unlocked, so "Normal" in creative means normal game play, and Normal in survival means Normal mode.

No, because that's my choice to play on survival. This is a sandbox game, locking away features and making them completely unobtainable until you reach some stupid, neigh impossible to get goal like getting 10,000 EXP is just stupid. The Nether and the End are hard to get to, yes, but give me 4 hours and I can get them. But 10,000 points without dying in lava or drowning, or being blown up by 2 creepers concurrently, or being killed a ways off, all of which reset your EXP with no chance of getting them back, is a really stupid idea. When Notch made achievements, he said that he didn't want them to be some grueling, stupid, repetitive task like chopping down 10,000 trees. This is just that, but on steroids, and with features being denied to you if you can't succeed.

I suggest Pig Mode: Make 1 million points on all modes and you can play as a pig.
