I have a mod proposal for some mod maker with ambition, and a will to change the game of Minecraft forever.
Minecraft Universe (MU)
Premise:
Think: Stargate
Allow players who are running the mod to build a new kind of portal in the game (an MU Portal). This portal would allow a player to step through and join with another minecraft server that is currently up.. and also currently running the MU mod.
When a player stepped through the portal, they would connect to the new server, and they would have whatever items they had on their person (equipped or inventory). While on this new server, returning through the same portal they emerged from, would return them to the server from whence they came. Alternately, they could find or build another MU portal, and step through that new portal to connect to some new server in the MU portal network.
MU Portals would require some appropriately complex item that acts as a power source for the Mu portal. Without power, an MU portal would not function.
An MU portal created on one world, would attempt to find a connection with another "free" mu portal somewhere else in the MU server network. If there was no "free" mu portal available, then it would itself be flagged as free, and the next portal created somewhere (on another server) within the MU network, would attempt to link with that portal.
With this design, portals are able to be deactivated to stop visitors from another world, and thus all portals are bi-directionally consentual
How could this possibly work?
Part 1: Web Application Component.
A centralized web application could act as the registrar of MU enabled minecraft servers. Each would could be registered automatically via the mod, or perhaps manually via the server's admin.
Once registered, the central app would retain knowledge about the server's name, address, up-status, etc. It would also maintain a record of how many powered "free" MU portals exist on each server.
Servers might be allowed to run their own mods, or might have to be a vanilla SMP configuration (except the MU mod of course) as implementation might dictate, though an attempt to allow server mods would be optimal.
Part 2: The Mod
On a server running the mod, a player could construct some new MU portal similar in concept to a nether portal. They could also create some powering device. Upon construction, the portal would attempt to obtain a fixed address to a particular server by finding the next free mu portal in the network and for a world that isnt already linked to the origin world. While the mu portals on both sides are powered, the portals go active, and players could step through to the other world. If power on either side is pulled, the portals go inactive.
If the portal on either end was destroyed, then the link would be broken, and the portal would be useless. A new portal would need to be constructed to create a new link to a new world (possibly the same one).
When a player passes through a mu portal, if the corresponding server is down, or if it is full (player cap reached), the player is notified, and they cannot pass through.
Some Issues to think about
A refinement could also be that the central app might also know each player's last connected server, for the purposes of re-establishing contact with a server the unexpectedly drops.
A refinement of MU Portal construction might be to allow users to specify an address in the MU server list, so as to construct a portal directly to the world of their choice. (dialing device anyone?)
A technical or gameplay limitation might have to be made to allow server admins to determine if players coming through an MU portal into their world are allowed to bring items or not. They may have to be naked to prevent griefing.

I read the first part and already knew it's impossible. You can't import items from one server to another, it's just physically impossible.
I don't think you can teleport players from one server to another, either.
I had figured this might be impossible. But simply choosing to connect to another server might be enough. You enter naked... consider it a side effect of the transport.
As for connecting to another server. If the game supports connecting to a server.. there would have to be *some* way to change the server you're connecting to. Though it may be inelegant. I admit I know nothing of the mod API exposed to devs.

There is actually a plugin that lets you warp to other servers.
