I think Soul Stones would be a cool new item to add into the game.
The Soul Stone could be made of 1 ender pearl, some zombie flesh and 1 gold ingot.
The Soul Stone Saves the player from death and gives them 3 hearts, 3 hunger back and a 3 second damage immunity. Once this effect happens the soul stone is automatically consumed.
The Soul Stone's passive effect heals half a heart every 10 seconds, minus 1 second per Soul Stone in your inventory.
The Soul Stone can only stack to 5 and all soul stones have a 20 second global cooldown and only 5 can be carried in your inventory at one time.

In any case, OP, give credit to the original source for this idea. It came out of the Equivalent Exchange mod. All you did was change it slightly.
On another note, there is no way to restrict how many are in the inventory, so they shouldn't be stackable at all.
As for reviving, I think it's lazy gameplay in general. The only thing that justifies it slightly is death from creepers or enviromental damage. In the later case it doesn't even help except in the case of falls, as you will still be taking damage and just die again. For most purpose, adding instant healing potions would be more useful. For instant/enviromental damage? My counter-offer:
The Phoenix Crest = Blaze Rod = Ender Pearl
This item would be worn in an additional item slot. When a person wearing one dies, they respawn with their inventory, minus the crest itself. Experience is still lost. (Alternately, they could respawn with experience but not items, or with half experience and some items, selected at random.)
