Two teams of 4 players (Maybe more?) Would spawn on opposite corners of a map,
I'd say around 500 by 500 to 1000 by 1000.
They would start out with nothing but their fists, and have to kill the other team.
They can start by getting better weapons and armor, like they normally would in a normal game.
Each person would have 1 life
I personally think this would be an interesting type of pvp, as team work would be required to out do the others.
Possible wooden starter tools to speed up game. And also not just opposite sides from from 0,0... randomize it a bit so teams don't know where each-other are.

Wooden tools would be great, but the randomization would slow the game down a bit, and maybe put the teams next to eachother.

I might set this up on a separate world on my server.... and put perma death on just that world....

I mean manual randomization.... Tournaments likely wont be starting every 2 seconds... Just have an op pick to points on map.
Plus, one simple randomizing calculation cannot lag a server when my puny server does fine with logblock update a database with every block change.
